AGREEMENT
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
AIR QUALITY DEPARTMENT
AND
MADISON ELEMENTARY SCHOOL DISTRICT #38

Purpose: Amend Federal Diesel Emissions Reduction Act State Clean Diesel Grant
Program Agreement for a period of six months (supply chain issues)

Contract Number: C-85-22-031-X-00

Funding Amount: No Change

Contract Effective Date: Upon Final Signature

Contract End Date: March 31, 2023

Maricopa County by and through its Air Quality Department (“County”) and the Madison Elementary School
district #70 (“MESD”) entered into an Agreement to extend the original grant period for an additional six
months.

Effective upon signature, the Agreement referenced above is hereby amended as follows:
Pursuant to the Agreement Terms, amend the following:

9., Agreement Term: The initial term of this Agreement shall commence on the Effective Date and end no
later than March 31, 2023.

The foregoing paragraphs contain all the changes to the Agreement. All other tetms and conditions of the
original Agreement as amended and approved remain in full force and effect and as agreed to by the Parties.
The Amendment shall be effective upon approval and signature by the Parties.
