AMENDMENT NO. 2 TO THE
CONTRACT
BETWEEN MARICOPA COUNTY
ADMINISTERED BY ITS HUMAN SERVICES
DEPARTMENT
AND
HUMAN SERVICES CAMPUS, INC.
I. Maricopa County (“County”) by and through the Maricopa County Human Services Department
and Human Services Campus, Inc. (referred to as “Contractor”) entered into a Contract on or
about January 1, 2019, for the provision of emergency weather relief accommodations for
individuals experiencing homelessness at the Human Services Campus. The County provided the
Subrecipient with $3,042.12 in FY19 Maricopa County General Funds for these services. The
County and the Subrecipient may be referred to individually as “Party” or collectively as “Parties.”
The parties executed Amendment No. 1 on or about June 19, 2019 to extend the contract
expiration date from June 30, 2019 to June 30, 2020.
II. The Parties agree to enter into this Amendment No. 2 to the Agreement to amend the Agreement
as follows:
A. The County shall provide Human Services Campus, Inc. with $100,000 for the period of
July 1, 2019 to June 30, 2020.
B. The County shall reimburse the Human Services Campus, Inc. at a cost not to exceed
$1,000 per night.
III. The County shall reimburse the Contractor on a Net 0 payment basis.
IV. This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs contain all the changes
made by this Amendment. All other terms and conditions of the original Agreement remain the
same and in full force and effect as approved and amended.
