Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization's history, mission and goals. Established in 1968, the Civitan Foundation originated as a community effort to provide a recreational camp program for children and adults living with a developmental disability in Williams, Arizona. In response to an ever-increasing need for year-round and Valley-based programming for the disabled, in 2003, Civitan expanded its quality service to include the Phoenix Metropolitan area. Now a Qualified Vendor with the Department of Economic Security/Division of Developmental Disabilities, Civitan is authorized to provide respite, habilitation, attendant care and a day program service. In 2013, Civitan Village was created. The Village is committed to innovation that removes barriers while opening new doors of opportunity for children and adults living with disabilities. Opportunities include: vocational and employment training, specialized activities in the areas of media, visual & performing arts, culinary/catering, sports & health, technology, and a summer food service program for community low-income youth during the summer months. Civitan's mission is to provide our community an accessible, safe and affordable environment while delivering superior life experiences to enhance the quality of life for children and adults with developmental disabilities. Civitan is committed to leading Arizona in the provision of unmatched, innovative, accessible, and affordable services for children and adults with developmental disabilities--becoming the foremost visionary and resource for this valuable community. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality's role in this project. If the applying organization is a municipality governmental sub-division please enter the text "Not applicable." Civitan Works closely with the state of Arizona, Maricopa County, and several municipalities through the Department of Developmental Disabilities and the Department of Education. Civitan receives grant monies to deliver workforce development, food and nutrition, and various other care programs for individuals with developmental disabilities throughout the state, all year. 3. Describe the proposed project and your plan to implement. Transitioning into Employment is an educational work readiness program for adults with intellectual and developmental Gila River Indian Community Grant Application Grant Cycle 2019 disabilities that desire to be employed. It is specifically designed to educate and train individuals for a variety of work environments. The training and work activities take place in seven separate vocational tracts (Media Design; Fine Arts; Culinary; Retail (Thrift Store); ECommerce; Janitorial; and Secretarial). The program consists of four phases: assessing career aspirations and current skill level; adapted classroom educational instruction; on the job training; and, job placement.TEP provides students with a broad spectrum of disabilities (down syndrome, cerebral palsy, autism spectrum disorders, intellectual disabilities) the skills to secure and maintain gainful employment. Program participants are expected to gain and demonstrate job readiness skills and be referred for employment upon program completion. 1) At least 70% of participants completing twelve (12) months of participation, or their individualized training schedule, whichever comes first, will be referred for community integrated employment. 2) At least 10% of participants participating in this program will be referred for competitive employment without supports. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. Civitan Foundation serves people marginalized and underestimated by the majority of society. However, like the Gila River Indian Community, Civitan believes in working with others, all others, and in the dignity of work. Civitan enhances the quality of life for individuals with developmental disabilities by providing pathways to meaningful work, and earning competitive wages. 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Young and older adults with development disabilities are often discouraged or discriminated against when it comes to employment opportunities in the community. Employers may be missing out on the skills and talents of people with disabilities because of assumptions and stereotypes that they aren't even aware of that can impact their hiring decisions. The unemployment rate among individuals with developmental disabilities is 85%. These individuals tend to acquire jobs that are disability typical, and are paid less than minimum wage. Our students, however, are seeking and acquiring nontraditional employment. An investment of $15,000 from the Gila River Indian Community will provide the necessary support to ensure one year of programming and related program capacity building for the Transitioning into Employment Program (TEP), serving 90 individuals with developmental disabilities, creating sustainable jobs within the community. 6. List three proposed outcomes for this project to be accomplished during the grant term. 1. Each program participant is expacted to gain and demonstrate job readiness skills and be referred for employment upon program completion. Gila River Indian Community Grant Application Grant Cycle 2019 2. At least 70% of participants completing 12 months of participation, or their individual training schedule, whichever comes first, will be referred for integrated employment. 3. At least 5% of participants participating in this program will be referred for competitive employment without supports. 7. Define the project as a new or continuing program. Has GRIC previously funded this project? This is a continuing program. 8. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. The TEP is an existing, year-round program with no discernable end or beginning. Individual time in the program may vary, but typically individuals remain in the program for multiple years. 9. Identify other organizations, partners or funders participating in the project and their roles. Current Employment Training Partners Include: • Gateway Academy • Mentor Kids USA • 32i'd & Shea • Coffee News — Newspaper delivery work program • Reach Beyond — a non-profit organization — production work program • AAEC High School- school cafe • Skytouch- sell weekly sandwiches & snacks • Summer Food Service Program for the ADE (Mentor Kids, Lincoln YMCA, Thunderbird Arts Center, Phoenix Center for the Arts, ARCH) • Phoenix Union HS's Upward & Onward Program at our retail store - Flerish Current Funding Partners Include: • Phoenix Suns Charities - $50,000 for Summer Food Service Program, Catering training and transportation efforts. • Fry's Kroger Foundation - $10,000 for employment training • Maricopa Association of Governments WAG) — $15,556 for transportation and vehicle safety equipment 10. Would you be able to implement the proposed plan if your organization received partial funding for this project? Civitan Foundation takes its role in the community seriously, as a result it has worked diligently to diversify and Gila River Indian Community Grant Application Grant Cycle 2019 strengthen its revenue sources for programs like the Transitioning into Employment Program. Although, full funding from the Gila River Indian Community would ensure continued, uninterrupted service and employment to 90 individuals, even partial funding will ensure service and employment for students. With an investment of $10,000, the Gila River Indian Community would serve 70 students, and with $5,000; 35 11. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. Sustainability is one of Civitan's greatest strengths, where we have steadily grown our operations over a heralded 50- year period, diversifying revenue streams through valued relationships with local, statewide and national funders, and demonstrating resilience in times of Government cuts. Careful attention has been paid to growing our program in manageable increments over time to maintain our high-ratio of staff- and volunteer-to-camper, as well as overall quality of experience. This lends itself to well-run, financially-stable operations where any loss or discontinuation of funding can be managed. Civitan is one of the most qualified vendors for DOD/DES in Arizona, and has a robust grants program. However, it is Civitan's intention to cultivate a relationship with Gila River Indian Community beyond the period of an award, as well as continue to forge strong relationships with other funders, individuals, and private and public groups to support our growth over the years to come. 12. Describe your plan to document progress and results. Civitan conducts an initial vocational/career interest inventory with participants that desire to learn the skills for employment. When participants have mastered the classroom, they are placed in a Civitan center based job in the area they trained; (culinary, media, cleaning crew, car/van washing, receptionist, piece work, and SNAP outreach worker). Employees are given a monthly work schedule and they sign in and out daily and job coaches monitor hour recordings and turn them into the bookkeeper. Civitan's Bookkeeper calculates work hours and employees are paid on the 10 and 25 of each month. More than 50% of program participants that are employed, have direct deposit. Every six months, employees go through a time study to qualify for a pay increase. Civitan ensures that direct service staff is trained in developing and teaching the employment related skills identified in the Program Objectives. Staff will complete this by: * maintaining a copy of each participant's current planning document; * keep daily progress notes for each participant in the program; * submit quarterly individualized progress reports and submit to Division of Developmental Disabilities; * Keep a record of each participant's attendance and number of hours worked; * Each participant shall successful pass an assessment at the end of each phase; and * Civitan will maintain data that demonstrates full compliance with all programmatic and contractual requirements of the Department of Labor and Division of Developmental Disabilities.
