CONTRACT NO: C-86-13-073-3-01 AMENDMENT NO: 1 AMENDMENT TO INTERGOVERNMENTAL AGREEMENT (IGA) Between MARICOPA COUNTY by and through its DEPARTMENT OF PUBLIC HEALTH, DENTAL SEALANT SERVICES and PHOENIX ELEMENTARY SCHOOL DISTRICT I. The above named IGA is hereby amended as specified below. A. Extend the term to begin July 1, 2019 and end June 30, 2023 unless terminated by either party. This is the maximum extension allowable. II. All other terms and conditions of the original IGA shall remain in full force and effect.
