Title: Overdose Data to Action
National Center for Injury Prevention and Control (NCIPC)
The purpose of this budget sheet is to assist the applicant with the budget planning process. For specific
details about the strategies, please refer to Section iii. Strategies and Activities within the NOFO. The
budget details the funding amounts for the required strategies and the budget if all optional activities and
fastest tiers are selected. Please use Appendix 1, Section 4 to determine your surveillance budget.
There are two required components of this award, a surveillance component and prevention component.
Within the surveillance component there are three strategies, (1 - 2 are required for state recipients, D.C.
and Puerto Rico, and 3 is required for all recipients). The prevention component has 7 strategies (4 - 7
are required, 8 -10 are optional). Please Note: Surveillance component funding should only support
surveillance activities (Strategies 1-3), while the prevention component funding should only support
prevention activities (Strategies 4-10). The budget narrative should clearly distinguish between funds
