AMENDMENT No. 1 TO LEASE AGREEMENT
BETWEEN
VEDRES FAMILY INVESTMENT PARTNERSHIP, LP, LESSOR
AND
MARICOPA COUNTY, LESSEE
RECITALS
A. Lessor, Vedres Family Investment Partnership, LP, a California limited partnership, and
Lessee, Maricopa County, a political subdivision of the State of Arizona; are parties to that
certain Lease Agreement No. L-7466 dated August 12, 2016 and ratified July 31, 2017, (the
“Agreement”). The Agreement is for 4,243 rentable square feet of clinic and office space
located at 2737 W. Southern Avenue, Suite #10, Tempe, AZ, 85282 (the “Premises”).
B. The current term of the Agreement expires on September 30, 2021.
C. Lessor and Lessee (together, the “Parties”) now mutually desire to enter into this Amendment
No. 1 to the Agreement (“Amendment No. 1”) to amend the Agreement to clarify certain terms
and append the Agreement.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other good and valuable
consideration, receipt and sufficiency of which is hereby acknowledged, the the Parties agree to
amend the Agreement as follows:
1. Section 3 of the Agreement is appended as follows:
3.1.1 Rent. Rent is due and payable on or before the seventh (7th) day of each month
that rent is due (the “Due Date”).
3.1.2 Invoicing. Lessor shall submit to Lessee on or about the twentieth (20th)
day of each month preceding the Due Date an invoice (the “Invoice”) to the
email address in Section 3.1.3 hereafter with the following information on it:

3.1.3 Late Payment. If Lessee fails to pay any installment of rent within 5 (five) days
after the Due Date, then Lessee shall pay to Lessor, only after written demand
by Lessor, a fee equal to ten percent (10%) of the unpaid amount that was due
on the Due Date. Lessor shall give notice or make demand at the following
electronic address:

4. The effective date of this Amendment No. 1 is the 15th day of the month following approval by
the Maricopa County Board of Supervisors.
5. The foregoing paragraphs contain all the changes made to the Agreement by this Amendment
No. 1. All other terms and conditions of the Agreement remain the same and in full force and
effect.
