THE PURPOSE OF THIS AMENDMENT IS TO:
1. Agreement Term.
Contract is hereby extended to September 30, 2021.
2. Agreement Terms, Section 5, Amount of Agreement.
The following is hereby added to Section 5:
“ADEQ authorizes and is to provide FY19 funds in the amount of $276,588 to Maricopa County for use as matching funds
under the Arizona State Clean Diesel Grant Program. At the start of each fiscal year, ADEQ will amend contract to provide
a new Scope of Work Workplan and a new funding Amount. Any changes or modifications to any part of this Agreement
shall be modified only by written Contract Amendment.”
3. Scope of Work.
Scope of Work FY2019, Exhibit A, is hereby incorporated into this Agreement (5 pages).
4. Agreement Terms, Section 15, Notices, Correspondence, Reports and Invoices.
Replace Section 15.2 with the following:
