Independent Contractor Agreement
Contract No. 07012018-19
The Independent Contract Agreement dated July 1, 2018, between Arizona Community Action
Association (ACAA) and Maricopa County, administered by its Human Services
Department (hereinafter “Contractor”) to conduct application intake and eligibility
determination for utility bill assistance and deposits is hereby amended as follows:
Purpose of the Amendment:
1. To increase the Arizona Public Service Crisis Bill Assistance Program funds by the
amount of $ $17,706.11 for the Total Allocation.
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources:
Arizona Public Service (APS) Crisis Bill Assistance Program: To increase the Direct
Service amount to $99,074.17 and increase the Program Delivery amount to $19,814.83.
The new allocation for APS BA will be $118,889.00 (Direct Service + Program
Delivery).
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in
place for the duration of the original Agreement.
