Exhibit 2b (Deliverable Acceptance Statements), DAS 5 (Emphasys SymPro Implementation Complete) shows the
price as $213,785.43. This Change Request (CR) is issued to correct the price information of Exhibit 2b, DAS 5. The
correct price is $157,698 (as shown in Exhibit A).
Recommended Solution:
• Replace the DAS 5 in Exhibit 2b with the corrected DAS attached to this CR.
Assumptions:
• The price correction of the Milestone 5 (Emphasys SymPro Implementation Complete) from Exhibit 2b is the only
change requested. This change matches the price in Exhibit A.

1.5 MS Dynamics Implementation Milestone #5 — Emphasys Sympro Implementation
Complete
Purpose:
The purpose of this acceptance form is for the Client to sign off on the Emphasys Sympro implementation phase and
agree that TRTA Gov should proceed to the next phase through the life cycle of this project, and accept billing for this
phase.
Acceptance Criteria:
1. TRTA Gov has validated debt information and reconcile to current system.
2. TRTA Gov has updated and validated prices (Fair Market Value) using Custodian Bank file.
3. TRTA Gov has validated reports functionality.
4. TRTA Gov has completed generation of test file and validation of Journal Entries.
5. TRTA Gov has delivered initial converted database.
6. TRTA Gov has identified users are trained and fully functional with the GL interface process.
7. TRTA Gov has generated files in SymPro system and loading and validation of Journal Entries into MS
Dynamics System.
8. TRTA Gov has facilitated County review of all named deliverables in above criteria and conducted a
milestone review meeting
This service was completed on:
Client response period is 10 business days. After that time, this deliverable will be considered accepted and ready for
billing unless otherwise documented in a formal response to TRTA Gov with detailed rational for rejecting of this milestone.
Rejection of a milestone will result in immediate escalation and halt the project for further review. See Appendix 2-A:
Deliverable Acceptance Process for guidelines related to deliverable acceptance.
We, the undersigned, agree that this work is complete and that under the conditions of this Statement of Work, Master
Agreement # AZ2015.001 and Schedule for Master Agreement # AZ2015.001.01 the County will be billed $157,698.00
upon signing this acceptance. It is agreed to proceed to the next phase of this project.
