Total grant amount request 10000 1 / 1 2017 Tohono O’odham Nation Grant Application Submitted by Arizona YWCA Metropolitan Phoenix a) Explain the perceived need and explain how the proposal will address that need. In Arizona, one in twelve seniors lives in poverty and one in six seniors face hunger. For the past forty-three years the YWCA Senior Meals program have served as a beacon to seniors and disabled throughout Glendale and El Mirage. Nearly half of our clients, 47%, live at or below 100% of the Federal poverty level. The YWCA provides the ONLY Meals on Wheels program that delivers meals for Glendale and El Mirage, and nutritious meals in a dining room setting at four locations, in all providing 105,000 meals. We also offer nearly 11,000 rides free-of-charge sponsored by the YWCA through the Dial-a-Ride program. In addition, more than 1,300 activities are held each year. The activities provide socialization, special events, community resources, supplemental food programs, health fairs and screening to address loneliness and isolation of our most vulnerable population, the seniors and disabled. The YWCA provides the ONLY Meals on Wheels program that delivers over 68,000 home delivered meals for Glendale and El Mirage, provides more than 37,000 nutritious meals in a dining room setting at four locations, distributes 4,700 frozen weekend meals, and provides free of charge 11,000 rides sponsored by the YWCA through the Dial-a-Ride program. In addition, more than 1,300 activities are held each year. The activities provide socialization, special events, community resources, supplemental food programs, health fairs and screening to address loneliness and isolation of our most vulnerable population, the seniors and disabled. Activities: 1) Serve one nutritious meal each weekday at one of three senior center locations and once a month at an additional senior center. 2) Registered dietitian approves all meal menus. 3) Provide transportation to and from each senior center location for lunch participants. 4) Provide meal delivery for homebound participants. 5) Schedule Health and Wellness presentations at two centers twice a month. 6) Provide socialization activities, special events, community resources, supplemental food programs, health fairs, and health screenings to address loneliness and isolation. Program Goal: Seniors and/or disabled adults eat at least one nutritious meal each week day. 1) Provide 37,000 meals to 700 unduplicated seniors and disabled adults in the Glendale area at one of four centers. 2) Deliver 68,000 nutritious meals to homes of 500 unduplicated seniors and disabled adults unable to visit one of our Centers. 3) Provide 11,000 rides free of charge to and from the Senior Center for seniors and disabled adults with Dial-A-Ride through YWCA. The YWCA works with other non-profit organizations to decrease hunger in Glendale's elderly community. We collaborate with the Area Agency on Aging to coordinate services. Other partnerships include: 1) the City of Glendale to provide recreation and socialization activities; 2) the Japanese American Citizens League to provide meals at their facility; 3) St. Mary's Food Bank Alliance shares maintenance food and supply items; 4) Gleaner's Brown Bag Program distributes food products to eligible participants; 5) Waste Not delivers excess food products, and 6) Midwestern University provides a Health Fair with medical students and professionals who assess clients general health, mental health, vision, and hearing. We are also in discussion with Glendale Community College regarding partnerships in a number of areas including student service projects, volunteerism, faculty presentations onsite at our Center, and GCC representation on YWCA Advisory Boards. b) Describe the population who will benefit from the proposal if approved. Page 1 of 4 2017 Tohono O’odham Nation Grant Application Submitted by Arizona YWCA Metropolitan Phoenix The Center serves the elderly and disabled adult population west of I-17 and between Bell and Camelback Roads. Last year, the YWCA provided 105,000 nutritious meals for elderly and adults with disabilities, and more than 1,000 seniors participated in our recreation, social activities and other services. We see an increase in multigenerational families -- instances of women in their late 60s and early 70s coming in for food with their mothers who in their late 80s. Meanwhile, seniors’ incomes decrease. For some of our seniors, the drivers who come to their homes are often the only people they see all day, or even all week. The value of human contact is invaluable as it helps keep their spirits up. The drivers often identify health problems and other needs the residents might have. Identifying and addressing health and social challenges is key to allowing them to remain independent and to stay in their own homes longer. The YWCA will not turn away seniors requesting food. Last year, the YWCA provided more meals than planned and contracted through local and state funding. In addition, our food costs increased nearly 20%, and our transportation costs are up 10% over last year. Our Centers ask seniors to contribute $2 per meal. However, our senior clients can only contribute an average of $0.67 per meal. This decrease in contributions, coupled with increasing demand for meals and higher costs leaves the program in great need of financial support from the community. c) Include a budget of the requested funds with the total amount requested clearly notated. See budget on final page d) List any other funding requests that have been made for this or substantially related proposals and provide the status of those requests.
