2017 Proposal to Tohono O’odham Nation
FSL Nutrition Program for Low-Income Seniors and Adults with Disabilities
a. Perceived Need and Program
Our Mission: "To provide home and community-based services and develop energy-efficient, affordable
housing, to promote health, independence and dignity for all."
FSL was created as a 501(c)(3) nonprofit organization in 1974 by the Diocese of Phoenix to address an
unmet need for housing and services for the elderly, persons with disabilities, and other vulnerable
individuals, such as adults with severe mental illnesses. FSL provides services irrespective of a person’s
race, color, religion, gender, sexual orientation, national origin, age or disability.
Valuing “home,” FSL’s programs and services support aging in place for adults who might otherwise
require admission to nursing homes due to age-related physical/ cognitive decline or lifelong
conditions. Programs include:
1. Congregate and home-delivered meals based in Peoria and Wickenburg and servicing communities
local to those cities;
2. In-home nursing and therapy visits, assistance with activities of daily living (bathing, dressing, etc.),
and chores;
3. Licensed Adult Day Health Services that provide medical care as well as social, therapeutic and
recreational activities;
4. Group homes provide in-home support services for behavioral health clients to assist them in
achieving their service plan goals and objectives;
5. Twenty-two (22) low-income housing communities for 800 elderly adults;
6. Home improvements provide low-income families with safe, sanitary, and energy-efficient homes.
FSL’s in-home and community-based programs serve 15,000 individuals annually in Maricopa County
while housing is offered throughout Arizona.
Community Need
FSL’s Nutrition Program addresses a widespread problem in the U.S. – food insecurity in our senior
population and its consequences. Thirteen percent (13%) of seniors in Arizona 65+ years of age live in
poverty, the third highest percentage among all 50 states in the U.S. (Kaiser Family Foundation, 2013)
Hunger and food insecurity are directly associated with poverty. (USAID, 2015) Thirty percent (30%) of
households with seniors must choose between paying for food or medical care. (Hunger in America,
2010)
In 2012, there were 3.7 million senior citizens who were diagnosed with malnutrition according to the
American Academy of Family Physicians. Feeding America reports that seniors aged 60 and over are
particularly vulnerable to the health consequences of food insecurity compared to other adult age
groups and suffer the following consequences when compared to their food-secure peers:
•53% higher rate in heart attacks
•52% more likely to develop asthma
1 | Page

2017 Proposal to Tohono O’odham Nation
FSL Nutrition Program for Low-Income Seniors and Adults with Disabilities
•40% more likely to experience congestive heart failure
•22% experience limits in their Activities of Daily Living, such as bathing, dressing, etc.
Additionally, choosing between food and medication is dangerous and our elderly are forced to make
this decision far too often. According to the American Medical Association, research studies show that
insufficient income contributes to upwards of 50% of senior citizens’ failure to follow their prescribed
medication regimens and this noncompliance is responsible for anywhere from 20-30% of hospital
admissions of those over the age of 65 years old.
FSL Nutrition Program for Low-Income Seniors and Adults with Disabilities
To alleviate food insecurity in Peoria and Wickenburg, two communities that are northwest of Phoenix,
FSL has operated multi-purpose senior centers since 1979 that focus on food distribution – congregate
meals, home-delivered meals, and food pantries. The Wickenburg Wise Owl Senior Center and Peoria
Community Center have on-site, full-service professional/industrial kitchens that serve congregate
meals Monday through Friday, and deliver hot meals weekdays with frozen meals delivered on Friday
for weekends.
In addition, the Wickenburg Center operates a mobile food pantry to assist vulnerable populations
access more fresh fruits and vegetables. A collaborative project with St. Mary’s Food Bank Alliance, the
pantry is held on the 4th Tuesday of each month. The food bank delivers between 8 and 10 pallets of
fresh produce to the center where staff and volunteers separate and package all the food for
distribution. Last fiscal year, we distributed 88,000 pounds of food through the mobile food pantry.
Our meals are prepared under strict guidelines and include a 2-3-ounce portion of protein, 2- ½ cup
servings of a vegetable, 2 ounces of a grain and either six ounces or ¾ cup of fruit. A registered
dietician reviews and approves all menus prior to their execution. All employees and volunteers are
required to secure a Food Handlers Card by completing online training identified by Maricopa County
for food handlers. We receive routine inspections by Maricopa County Environmental Services to
ensure safety in food preparation and service and have always received exemplary reports.
In addition to delivering meals, our drivers also perform a welfare check. They are often the only
human interaction our clients have. We serve a vulnerable population that is often isolated and which
suffers from multiple health conditions
Goals
Last fiscal year ending 6/30/16, FSL served 52,138 meals to 681 unduplicated seniors and adults with
disabilities. FSL projects to serve a minimum of 50,000 meals, the majority of which are enjoyed in a
congregate setting, the upcoming fiscal year. The goals of the program are to:
1. Allow low-income seniors and adults with disabilities to age in place;
2. Maintain health to prevent admission to institutions;
3. Prevent hunger.

b. Describe the population who will benefit from the proposal if approved.
The seniors in the communities we serve are especially vulnerable. According the 2010 U.S. Census,
31% of Wickenburg’s total population is 65+ years old and 18.9% of residents live at or below the
federal poverty level. Additionally, 52.6% of Wittman residents, a neighboring town served by
Wickenburg, live in poverty. In Peoria, 34% of residents live at or below the federal poverty level and
receive Supplemental Nutrition Assistance Program (SNAP) benefits.
Adults with disabilities who are homebound rely on home-delivered meals in order to age in place.
They are unable to prepare their own meals and FSL’s Nutrition Program provides access to nutritious
lunches every day of the week. For many families, access to home-delivered meals allows caregivers to
work, secure in their knowledge that their loved one at home will receive a nutritious, hot meal while
they are absent.
