a) Explain the perceived need and explain how the proposal will address that need.
In 2010, chronic disease – including cancer, heart disease, chronic lower respiratory disease,
Alzheimer’s disease, diabetes, and chronic liver disease & cirrhosis were responsible for seven
of the ten leading underlying causes of death in Arizona. When combined, these chronic
diseases were responsible for more than 29,500 Arizona deaths in 2010. (Arizona Health Status
& Vital Statistics, 2010).
In addition to 9,719 deaths that had heart disease assigned as the underlying cause, another
6,789 deaths had diseases of the heart assigned as the other than underlying cause. The sum of
these two counts (16,508) is the total number of deaths that had any mention of diseases of
the heart on the 2010 death certificates (Arizona Chronic Disease Strategic Plan, 2012 - 2015).
Currently, seven out of the ten leading causes of death are attributed to a chronic disease and
86% of the annual health care spending is related to chronic disease. As of 2012, about half of
all adults—117 million people—have one or more chronic health conditions. One in four adults
has two or more chronic health conditions (CDC website).
The Stanford Model of evidence-based self-management programs, also known as Healthy
Living workshops, is a proven intervention that can help Arizonans lead longer and healthier
lives. Unfortunately, many do not know about this opportunity or have close proximity within
our community. The Healthy Living Initiative of Maricopa County will help bridge the gap to
access to self-management workshops by empowering community partners to deliver this
evidence-based program to multiple locations within Maricopa County.
The purpose of The Healthy Living Initiative of Maricopa County is to implement and expand
self-management programs to populations who have a need for Chronic Disease education with
the combined goal of improving health, increasing the self-efficacy of completers, reducing
costs associated with healthcare and reducing preventable hospital readmissions within
Maricopa County.
Goal #1: Train 30 lay-leaders to facilitate Healthy Living workshops per grant year
Goal #2: Create partnerships that result in the planning and facilitation of 30 Healthy Living
workshops per grant year (self-management workshops can be provided in both English and
Spanish).
Goal #3: Provide technical assistance for workshop implementation that includes activities
related to workshop logistics and support including workshops books and nutrition support.
Maricopa County Department of Public Health will work closely with community partners to
implement the initiative. The Arizona Living Well Institute (AZLWI) is the state endorsed vendor
of self-management workshop implementation. They provide lay-leader training and workshop
fidelity. MCDPH works closely with this organization to ensure participants have a positive
experience while engaging in the program.
2

Work will begin immediately with the creation of partnerships and lay-leader training activities
to support workshop volume that is projected.
Step 1: identify and train 8 lay-leaders each quarter
Step 2: plan and implement 8 Healthy Living Workshops each quarter
b) Describe the population who will benefit from the proposal if approved.
The workshops are promoted to people who have a chronic disease, are at risk for a chronic
disease, or are the caregiver for a person with chronic disease. Implementation of 30
workshops annually will provide approximately 360 individuals with access to an evidence
based program that improves overall health while at the same time reducing healthcare costs
and hospital readmissions.
The Healthy Living Initiative of Maricopa County will provide Healthy Living workshops to over
360 individuals in each year of funding.
c) Include a budget of the requested funds, with the total amount requested clearly notated.
The proposed budget for each year of the initiative is as follows:
Proposed Budget Expense Amount Requested Total Budget for the Initiative
Lay Leader Training $9,600 $9,600
Workshop Books $12,800 $12,800
Workshop Coordination Stipend $16,000 $16,000
Nutrition Support (fresh produce) $6,400 $6,400
Total $44,800 $44,800
d) List any other funding requests that have been made for this or substantially related
proposals and provide the status of those requests.
Fort McDowell Yavapai Nation, pending.
e) Indicate whether the entity has already received a 12% distribution from the Tohono
O’odham Nation or other Arizona tribes, along with the amount and date awarded.
June 5, 2013, Tohono O’Odham, $50,000.00 (Parent Ambassador)
August 31, 2011, Gila River Indian Community, $150,000.00 (STD Mobile Services)
f) Identify a designated contact person and alternate contact person for follow-up, including
e-mail address, phone number, mailing address, and fax number for these persons.

g) Indicate which priority area (from the above list) the proposal falls under.
Health Care - The proposed initiative features self-management of chronic disease and diabetes
education workshops. These evidence-based workshops promote an increase in self-efficacy in
those who are at risk or are suffering from chronic disease and/or diabetes. The Stanford
Model of self-management workshops are designed to assist in improving the overall health of
participants.
h) If the proposal is from a non-profit organization, include the name of the city, town, or
county an agreement will be made with if funds are granted. A letter from the governmental
entity acknowledging its agreement to work with the non-profit is not necessary for the initial
proposal. However, it is necessary for those non-profits selected to receive grant funding.
