I. The above named contract is hereby amended as specified below:
A. Add two (2) additional sites to provide Community Health Needs Assessment.
1. Arizona Orthopedic Surgical Hospital, 2905 W. Warner Road, Chandler, AZ 85224;
2. OASIS Hospital, 750 N. 40th Street, Phoenix, AZ 85008
B. Increase Contract in the amount of $30,000 for a contract total not-to-exceed $150,000.
II. All other terms and conditions of the original contract shall remain in full force and effect.
