these incentives for completion of TB therapy among patients receiving treatment for disease or latent TB
infection.
Also attached are two forms. Use one form to document and monitor the recipients.. The .other is a template of
the monthly report (no names should be included, which should be submitted to thiS.affice on a monthly 'basis.
