CONTRACT NO: C-86-24-086-X-03 / PH NOI 230163 AMENDMENT NO: 4 AMENDMENT TO CONTRACT Between MARICOPA COUNTY by and through DEPARTMENT OF PUBLIC HEALTH and THE FAITHFUL CITY, INC. I. The above-named contract is hereby amended as specified below: A. Section 1.0 CONTRACT TERM, Subsection 1.1 shall be updated to reflect a new expiration date of August 31, 2027. B. Exhibit A – BUDGET shall be updated to include an additional $260,000 for the Year 4 Budget Period of September 01, 2026 to August 31, 2027, bringing the not-to-exceed amount to $1,040,000. C. Exhibit A – BUDGET shall be revised to include Exhibit A-1 Year 4 Budget & Budget Narrative (September 01, 2026 to August 31, 2027), beginning on page 3 of this amendment. D. Exhibit B, Scope of work, shall be replaced with Exhibit B-1, Revised Year 4 Scope of Work (September 01, 2026 to August 31, 2027), beginning on page 6 of this amendment. E. Exhibit D shall be replaced with the 2026 Notice of Award for Subrecipients, beginning on page 9 of this amendment. II. All other terms and conditions of the original contract shall remain in full force and effect.
