CONTRACT NO: C-86-24-086-X-03 / PH NOI 230163 AMENDMENT NO: 4 AMENDMENT TO CONTRACT Between MARICOPA COUNTY by and through DEPARTMENT OF PUBLIC HEALTH and ARIZONA ASSOCIATION OF COMMUNITY HEALTH CENTERS dba ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS I. The above-named contract is hereby amended as specified below: A. Section 1.0 CONTRACT TERM, Subsection 1.1 shall be updated to reflect a new expiration date of August 31, 2027. B. Exhibit A – BUDGET shall be updated to include an additional $271,148 for the Year 4 Budget Period of September 01, 2026, to August 31, 2027, bringing the not-to-exceed amount to $1,083,445. C. Exhibit A – BUDGET shall be revised to include Exhibit A-1, Year 4 Budget & Budget Narrative (September 01, 2026 to August 31, 2027), beginning on page 3 of this amendment. D. Exhibit B – SCOPE OF WORK shall be replaced with Exhibit B-1, Revised Year 4 Scope of Work (September 01, 2026 to August 31, 2027), beginning on page 8 of this amendment. E. Exhibit D shall be replaced with the 2026 Notice of Award for Subrecipients, beginning on page 11 of this amendment. II. All other terms and conditions of the original contract shall remain in full force and effect.
