C-86-25-049-X-00 I PH RFP 250031 AMENDMENT TO CONTRACT PH RFP 250031 BETWEEN MARICOPA COUNTY BY AND THROUGH ITS DEPARTMENT OF PUBLIC HEALTH AND AXIOM COMMUNITY OF RECOVERY The above-named contract is hereby amended as specified below: AMENDMENT 2 I. Section 1.0 CONTRACT TERM shall be updated to reflect a new expiration date of March 31, 2027. II. An additional $236,164 shall be added for the Year 2 Budget Period of 04/01/2026 - 03/31/2027 as outlined in Exhibit B-2, Year 2 Budget Sheet & Narrative. Any funds from Year 1 that have not been spent within the Year 1 Budget Period of 04/01/2025- 03/31/2026 will be forfeited and therefore excluded from the updated not-to-exceed amount of $472,328. Ill. Exhibit D-1, Scope of Work, shall be replaced with a revised scope of work and is now known as D-2. IV. Exhibit J, Work Plan, shall be revised to include Year 2 Timeline. V. Addition of Exhibit K, Year 2 Renewal Form. VI. Section 3.0 PAYMENTS shall be updated to include the following clauses: 3.1.1 Funding Sources When multiple funding sources are used concurrently (e.g., braided federal and local funds), the County shall specify the order of funding priority for invoicing and cost attribution, if requested by the Contractor. Unless otherwise stated, federal funds shall be applied first, followed by non-federal funds. 3.1.2 Funding Source Substitution The County reserves the right to substitute or combine funding sources, including federal, state, local, and/or other County-administered funds (such as Opioid Settlement Funds), for payment under this contract. Such substitution shall not constitute a material change requiring a formal amendment to the agreement, provided that the scope of work, reporting requirements, and performance expectations remain unchanged. The County shall make reasonable efforts to provide written notification of any funding source substitutions. This notice may include: • The effective date of the funding source substitution • The name and type of the new or additional funding source(s) • Any updated reporting requirements, templates, or platforms • Any applicable unallowable cost restrictions tied to the new funding • Any changes to the contractor's classification (e.g., subrecipient vs. vendor) Page 1 of 16
