PetSmart Charities Application Approved for MARICOPA COUNTY 
Dear Debbie,
Congratulations! The Everyday Adoption Center application you submitted to PetSmart 
Charities on behalf of MARICOPA COUNTY has been recommended for approval in the 
amount of $75,000.00.
The next step in the process will be for your organization to sign an electronic 
agreement. Within the next 14 days, the leadership contact listed on your application 
will receive an email notification via DocuSign. That email will contain the agreement for 
signature and must be signed by an individual with signing authority for your 
organization (e.g. President/CEO/Executive Director or other legally designated officer).
To ensure there is no delay in your agreement execution, please login to SmartSimple 
and review your Organization Profile which must include two contacts with titles. One of 
which must be designated as having legal authority to sign on behalf of your 
organization.
This grant is not considered fully approved until the agreement has been signed, 
returned and countersigned by PetSmart Charities leadership.
