Exhibit C
Scope of Work
1. Food Pantry Services
a.
Identify a minimum of 200 households, determined as “high frequency” users of food
pantry services to receive the following services:
i. Continued access to emergency food boxes (200 households)
ii. Nutrition education (180 households)
iii. Community resource navigation (180 households)
iv. Assistance enrolling in public benefits (50 households)
v. Transportation assistance (50 households)
b.
Achieve the following program outcomes:
i. 180 households will increase nutrition skills 80% of survey responses
ii. 180 households will demonstrate improved economic security 80% of survey
respondents
iii. 160 households will demonstrate improved physical health and wellbeing
(80% of survey respondents)
c.
Conduct follow up contact with clients at intervals of 3 months, 6 months, and 9
months following receipt of program services to assess for the attainment of program
outcomes and the need for referrals to other program services.
d.
Ensure that all services provided are culturally relevant and linguistically appropriate
to the population to be served.
2. Reporting Requirements
a.
TCAA shall submit programmatic reports to City as listed below:
i. Results Oriented Management and Accountability (ROMA/ROMA Next Gen)
Report no later than 10th business day after the end of each quarter of the fiscal
year to include the following information:
1. Unduplicated number of individuals and households served
2. Unduplicated number of individuals and households that attained
program outcomes
ii. The County/City reserves the right to add, remove, or revise reporting
requirements to meet program goals.
iii. Failure to submit required reports in the designated timeframe listed may result
in:
1. A forfeiture of payment if not submitted by the forty-fifth (45th)
calendar days following the end of the month.
2. A forfeiture of final payment if final program report is not submitted
within the designated time period determined by County/City following
the Contract term.
3. Staff Requirements
a.
TCAA shall:
i. Maintain documentation indicating key staff have received appropriate training
or hold appropriate certification/licensure in accordance with roles,
responsibilities and job descriptions.
ii. Ensure that staff and volunteers do not have any conflicts of interest in the
provision of services and management of programs.
iii. Provide staff and volunteers with supervision, equipment, materials, and
supplies necessary to perform contracted services.
iv. Ensure staff and/or volunteers do not provide direct services to clients unless
appropriate background checks and fingerprint clearance have been completed
with satisfactory results.
v. Maintain the Central Registry Background Check and Fingerprint Clearance
results and any related forms or documents in a confidential file for five (5)
years after termination of the contract. Upon request, TCAA shall make
available valid background check information to County/City.
vi. Ensure procedures are in place if staff/volunteer background or fingerprint
clearance results are unsatisfactory.
4. Safeguarding of Participant/Client Information
a.
TCAA shall observe and abide by all applicable State and Federal statutes, rules and
regulations regarding the use or disclosure of information including, but not limited
to, information concerning applicants for and recipients of contract services. To the
extent permitted by law, TCAA shall release information to the County/City Arizona
Department of Economic Security, and to the Attorney General’s Office as required
by the terms of this contract, by law or upon their request.
b. TCAA shall comply with the requirements of Arizona Address Confidentiality
Program, A.R.S. 41-161 et. seq., the County/City will advise TCAA as to applicable
policies and procedures adopted for compliance.
5. Child Support Services
a.
TCAA shall have a process to ensure that custodial parents in single-parent families
must be informed by the availability of child support services, and eligible parents
must be referred to the child support offices of State and local governments.
b. Grievance Procedures
i. TCAA shall establish a system through which applicants for, and recipients of,
services may present grievances and may take appeals about eligibility and
other aspects of TCAA’s work under the Contract. The grievance procedure
shall include provisions for notifying the applicants for, and recipients of,
services of their eligibility or ineligibility for service and their right to appeal
to the City if the grievance is not satisfied at TCAA’s level.

Certification Regarding Lobbying
Certification for Contracts, Grants, Loans, and Cooperative Agreements.
The undersigned, on behalf of Tempe Community Action Agency, certifies, to the best of his
or her knowledge and belief, that:
1. No Federal appropriated funds have been paid or will be paid, by or on behalf of the
undersigned, to any person for influencing or attempting to influence an officer or
employee of an agency, a Member of Congress, an officer or employee of Congress, or
an employee of a Member of Congress in connection with the awarding of any Federal
contract, the making of any Federal grant, the making of any Federal loan, the entering
into of any cooperative agreement, and the extension, continuation, renewal, amendment,
or modification of any Federal contract, grant, loan, or cooperative agreement.
2. If any funds other than Federal appropriated funds have been paid or will be paid to any
person for influencing or attempting to influence an officer or employee of any agency, a
Member of Congress, an officer or employee of Congress, or an employee of a Member
of Congress in connection with this Federal contract, grant, loan, or cooperative
agreement, the undersigned shall complete and submit Standard Form-LLL, “Disclosure
Form to Report Lobbying,” in accordance with its instructions.
3. The undersigned shall require that the language of this certification be included in the
award documents for all subawards at all tiers (including subcontracts, subgrants, and
contracts under grants, loans, and cooperative agreements) and that all subrecipients shall
certify and disclose accordingly.
This certification is a material representation of fact upon which reliance was placed when this
transaction was made or entered into. Submission of this certification is a prerequisite for making
or entering into this transaction imposed by Title 31 U.S.C. §1352. Any person who fails to file
the required certification shall be subject to a civil penalty of not less than $10,000 and not more
than $100,000 for each such failure.
