| am the trustee for the Richard J. Franks, Irrevocable Trust, (formerly t
Trust in which | was also the Successor Trustee) which is the owner of

he ownership was changed upon the death of the original trustee,
Patricia Franks on 2/3/23. | was the successor trustee. Upon the original trustees death,
the new trust was created and | worked with Maricopa County and the ownership was
changed to the Richard J. Franks Irrevocable Trust in 2024 with the new mailing address
noted below.

| have attached uncashed Check number 1071556, on August 15, 2025. It was Issued
on 8/14/24 and dated August 14, 2024 in the amount of $679.26 regarding the Qasimyar
settlement. This was sent to an old address recently, even though we had updated the
address, (that address is noted below). The check was just forwarded to me and |
received it on August 15, 2025. Also, it is made payable to the old trust and is over 365
days old so it can't be deposited.

| have attached a completed, notarized and signed Application for a Duplicate or Stale
Dated Warrant/Check Affidavit/Claim Form.

| request that you please cut a new check in the amount of $679.26, made payable to
the Richard J. Franks Irrevocable Trust. | have also attached the recorded Special
Warranty Deed, the Certification of Trust documents, Affidavit of Death of Trustee for

Change of Title To Real Estate & Grantee Trust Disclosure.
