Development of the site shall be in substantial conformance with the Site Plan entitled “Site Plan for KC Basketball“, consisting of 1 full-size sheet, dated July 16, 2025 except as modified by the following conditions. Staff may determine slight refinements to remain in substantial conformance with the approved site plan. Minor and major amendments to the site plan will be determined in accordance with Chapter 3 of the Maricopa County Zoning Ordinance. b. Development of the site shall be in substantial conformance with the Narrative Report entitled “Crandall Property - Youth Basketball Training”, consisting of 7 pages, dated August 2025 except as modified by the following conditions. c. The following Planning Engineering conditions shall apply: 1. Engineering review of planning and/or zoning cases is for conceptual design only. All development and engineering design shall be in conformance with Section 1205 of the Maricopa County Zoning Ordinance; Drainage Policies and Standards; Floodplain Regulations for Maricopa County; MCDOT Roadway Design Manual; and current engineering policies, standards and best practices at the time of application for construction. 2. The proposed construction appears to meet the requirements of the Maricopa County Zoning Ordinance Section 1205.7.7 and as such a site plan containing the information on the attached checklist (Form 718) is required. 3. Based on the conceptual design nature of the information submitted, changes to the site layout may be necessitated by the final design of the infrastructure. d. This special use permit is valid for a period of 10 years and shall expire on October 22, 2035, or upon termination of the use for a period of 90 or more days, whichever occurs first. All site improvements associated with the special use permit shall be removed within 90 days of such expiration or termination of use. e. Prior to occupying the existing residence or any portion thereof for any use associated with the Special Use Permit, the applicant shall obtain a Building Permit to retrofit the existing gym building to meet current commercial building code requirements as applicable and shall obtain a Certificate of Occupancy for the retrofitted building prior to occupancy. f. Operations shall be limited to the following: 1. A maximum of 12 non-family children can be engaged in training at one time. 2. August – May: Maximum of 5 training sessions per day Monday-Thursday, 3:30pm – 9: 00pm; site must be vacated by members of the public no later than 9:30pm. 3. August – May: Maximum of 8 training sessions per day on school breaks Monday-Thursday, end by 9:00pm; site must be vacated by members of the public no later than 9:30pm. 4. June – July: A maximum of 8 training sessions per day Monday-Friday, 7am – 4pm. 5. No scheduled training on Saturdays, only occasional make-up or one-on-one sessions. 6. No scheduled training on Sundays. 7. No outdoor training. 8. No use of the gym for other groups/sports. g. The following Rural-43 SUP standard shall apply: 1. Parking spaces: 9 total w/ 1 ADA space. 2. There shall be no on-street parking associated with the facility. h. Noncompliance with any of the conditions assigned to the approval of this Special Use Permit by the Maricopa County Board of Supervisors may be grounds for revocation in accordance with the requirements and procedures as set forth in the Maricopa County Zoning Ordinance. i. The property owner/s and their successors waive claim for diminution in value if the County takes action to rescind approval due to noncompliance with conditions. j. The granting of this change in use of the property has been at the request of the applicant, with the consent of the landowner. The granting of this approval allows the property to enjoy uses in excess of those permitted by the zoning existing on the date of application, subject to conditions. In the event of the failure to comply with any condition, and at the time of expiration of the Special Use Permit, the property may be considered for revocation to the zoning that existed on the date of application. It is, therefore, stipulated and agreed that either revocation due to the failure to comply with any conditions, or the expiration of the Special Use Permit, does not reduce any rights that existed on the date of application to use, divide, sell or possess the property and that there would be no diminution in value of the property from the value it held on the date of application due to such revocation or expiration of the Special Use Permit. The Special Use Permit enhances the value of the property above its value as of the date the Special Use Permit is granted and reverting to the prior zoning results in the same value of the property as if the Special Use Permit had never been granted. Willis Road is under the Town of Gilbert’s jurisdiction. Submit the TIS to TOG for review and approval. Provide approval status during building permitting process. d. The following C-2 CUPD standards shall apply: 1. Max. Height: 40-feet (no height setback from rural/residential required) 2. Min. Street-side Yard: 0-feet 3. Min. Any existing building or structure that are over 200 sq. ft. in size, over 8’ in height, or has utilities that lack an existing building permit must submit as-built permits for these structures. e. Prior to the issuance of any building permit on this site all Federal Patent Easements which impact this site must be abandoned. f. Use of adjacent land for overflow parking shall require a BLM lease agreement. g. The following Planning Engineering conditions shall apply: 1. Engineering review of planning and/or zoning cases is for conceptual design only. All development and engineering design shall be in conformance with Section 1205 of the Maricopa County Zoning Ordinance; Drainage Policies and Standards; Floodplain Regulations for Maricopa County; MCDOT Roadway Design Manual; and current engineering policies, standards and best practices at the time of application for construction. 2. Based on the conceptual design nature of the information submitted, changes to the site layout may be necessitated by the final engineering design of the site’s drainage infrastructure. 3. Detailed Grading and Drainage (Site Infrastructure) Plans must be submitted with the application for Building Permits. 4. Prior to any building permit issuance on this site, Drainage Waiver DRB250003 must be approved. 5. At the building permitting stage the berm around the retention basins must have a minimum width of 8’ and this shall be indicated on the grading and drainage plan. 6. At the building permitting stage the upstream invert of the existing culverts across the driveway in the New River Rd. right-of-way must be at or above the Retention Basin B’s stormwater elevation, which shall be indicated on the grading and drainage plan. 7. At the building permitting stage the grading and drainage plans submitted with the permit shall illustrate multiple spot elevations at the property line to show that the stormwater retention will stay on the property and shall not encroach into the right-of-way or public street. 8. At the building permitting stage, the culverts in the MCDOT Right-of-Way must be surfaced with concrete (RCP or RGRCP) which must be indicated on the grading and drainage plan. h. The following Maricopa County Environmental Services Department (MSESD) shall apply: 1. Per the Safe Drinking Water Act, any water system that supplies more than 25 people or 15 service connections per day for at least 60 days per year shall be classifies as a Public Water System (PWS). If this is a case on this site then a Public Water System application with a New Source Approval application shall be required and must be submitted to the MCESD’s Drinking Water Program. Moreover, a water quality analysis report shall be required with this submittal which is good within one-year of testing. 2. A Minor Plan Review application must be submitted for any existing system(s) if being utilized and if the applicant believes the existing system(s) are adequate in size to support the new use, otherwise, a new septic permit / installation may be required. 3. For any food service operations occurring on site, where food is offered, sold, or prepared from a fixed/permanent location the submittal of a Permanent/Fixed Food Establishment Permit application shall be required. i. Prior to issuance of a building permit, written confirmation will be required from the emergency fire protection jurisdiction having authority that the facility has been designed in accordance with their regulations and requirements, and that emergency fire protection service will be provided to the facility. Prior to issuance of the certificate of occupancy, local fire protection jurisdiction review and approval will be required. j. The following C-3 CUPD development standards shall apply: 1. There shall be minimum rear setbacks of 10’ regardless of adjacent zoning districts. 2. There shall be minimum side setbacks of 0’ regardless of the adjacent zoning districts. 3. There shall be a minimum 64 parking spaces for the existing uses on this site, 5% of which shall be reserved as ADA accessible parking spaces. 4. Driveways and parking areas shall be surfaced with alternative dust control methods, such as a quarter inch of decomposed granite or quarter inch of asphalt millings, or other methods deemed acceptable with the County Air Quality Department. ADA accessible parking spaces must be paved. 5. There shall be no required sight visibility triangles but clear sight-lines must be maintained in accordance with Maricopa County Department of Transportation requirements. 6. There shall be no required screening. 7. Maximum height for commercial freestanding signs shall be 25’. 8. Minimum distance between commercial freestanding signs on the same street frontage shall be 3’ between the existing south signs and 190’ between the existing north sign and existing south signs. 9. Any outdoor lighting on this site shall be in conformance with the provisions of Section 1112 of the County Zoning Ordinance. All outdoor lighting shall be shielded and placed to reflect light away from any adjoining rural or residential zoning district. k. Noncompliance with any of the conditions assigned to the approval of this Zone Change by the Maricopa County Board of Supervisors may be grounds for revocation in accordance with the requirements and procedures as set forth in the Maricopa County Zoning Ordinance. l. The property owner/s and their successors waive claim for diminution in value if the County takes action to rescind approval due to noncompliance with conditions. m. The amount of supplement #1 is $80,000, increasing the subgrantee amount from $270,000 to $350,000. Also extend the program period for one year, beginning October 1, 2025, through September 30, 2026. MCSO will reimburse YOUTURN HEALTH using grant funds awarded from the Arizona Criminal Justice Commission, Arizona Criminal Justice and Treatment Improvement Program designated for use in identifying, treating and supporting those impacted by drugs of abuse. The agreement and acceptance of this funding was authorized by the BOS (C-50-24-141-X-00) on February 28, 2024. The Subgrantee will perform tasks to include, but not limited to: provide a license to MCSO for educational content for the term of this Agreement, that includes Behavioral Health and Substance Use Disorder materials; provide Peer Coaching participants & their families access to Subgrantee’s entire learning materials platform; provide ACE Peer Coaching delivered by Subgrantee employees through text messages, phone calls and/or video calls; implementation and program management of the above mentioned tasks. This is not a mandated function although it supports the MCSO public safety mandate and provides benefit to the citizens by providing funding for the Impaired Driving Programs Coordinator to coordinate, strengthen and expand training initiatives to law enforcement personnel and certification as Drug Recognition Experts (DREs). SUBARU LOVES PETS PROGRAM GRANT AGREEMENT AND BUDGET ADJUSTMENT Approve the application and acceptance of the Subaru Loves Pets Program grant funds from The American Society for the Prevention of Cruelty to Animals (ASPCA) in the amount not-to-exceed $4,000. The grant provides funding for a mandated service, which is to support lifesaving objectives at the Maricopa County Animal Care & Control. Authorize the Animal Care and Control appointed authority, and/or designee, to accept the grant-related terms and submit all grant reports applicable under the grant. The grant award begins upon approval and ends on October 31, 2025. There is no grant award or signature required but by endorsing and depositing the grant award check, or accepting the electronic grant award deposit, the department agrees to the terms listed in the award agreement. In accordance with A.R.S. §42-17106(B), authorize the increase of revenue and expenditure authority in the Animal Care and Control (D790) Animal Control Grants Fund (573) Non-Recurring Non-Project (NRNP) appropriation group by $4,000 in Fiscal Year 2026. The grant allows a 0% rate or $0 for indirect costs. The Maricopa County Department of Finance has calculated the Fiscal Year 2026 indirect cost rate at 12.83% or $513.20. The recoverable cost rates for administering this grant are $0 and the Animal Care and Control will absorb the non-recoverable indirect cost of $513.20 into the department’s operating budget in the current fiscal year. This grant is non-recurring, and a cash or in-kind contribution is not applicable. There are no future or ongoing contributions required following the grant period. Funding for this agreement is provided by a grant from ASPCA and was competitively bid. The grant cannot be used in any way that decreases the allocation or budget of governmental funds for animal welfare purposes. This request will increase the grant budget appropriation in the Animal Control Grants Fund (573) to accommodate revenues and expenditures authorized by the grant. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation and, therefore, expenditure of the funds is not prohibited by the budget law. AMENDMENT TO MOU AND BUSINESS ASSOCIATE AGREEMENT WITH MERCY CARE Approve Amendment No. 2 to the Memorandum of Understanding (MOU) between Mercy Care and Maricopa County by and through its Department of Correctional Health Services (CHS). The purpose of this MOU is to establish a collaborative protocol for effective communication, coordination, and continuity of care for individuals eligible for services provided by Mercy Care who are also served by CHS. The purpose of this amendment is to revise Sections III-C-1, 2, 5, 6, and 8 of the MOU. All other terms and conditions remain unchanged. Mercy Care is also requesting signatures on both the Web Portal Agreement and the Business Associate Agreement (BAA). This agreement enables access to Mercy Care’s web portal to facilitate the retrieval of member/enrollee eligibility information. 64. SECURITY SERVICES BUDGET INCREASE Approve the following amendments for the FY2026 budget for personnel and equipment. The allocation of additional funds for security personnel will enhance protection measures and support evolving safety standards. Intel Analyst: the allocation of additional funds for an Intelligence Analyst is critical to enhance threat analysis capabilities and support strategic decision-making processes. Rightsizing Overtime: Adjusting the overtime budget is necessary to align expenditures with operational workload demands, thereby optimizing resource utilization and controlling costs. In accordance with A.R.S. 42-17106(B), approve the following amendments to the FY 2026 budget: Increase the expenditure appropriation in the Emergency Management (D150) General Fund (100) Operating (OPER) budget by $442,404 Increase the expenditure appropriation in the Emergency Management (D150) General Fund (100) Non Recurring (NRNP) budget by $278,480. Decrease the expenditure appropriation in the Non Departmental (D470) General Fund (100) Operating (OPER) budget in the “Unreserved Contingency” (4711) line by $442,404. Decrease the expenditure appropriation in the Non Departmental (D470) General Fund (100) Non Recurring (NRNP) budget in the “Unreserved Contingency” (4711) line by $278,480. Finally, authorize the Office of Budget and Finance to include the following changes to the FY 2027 budget baselines: Increase the expenditure in the Emergency Management (D150) General Fund (100) Operating (OPER) budget by $298,910. These actions will have a County-wide net impact of $0 and they do not alter the budget constraining the expenditure of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. CAPITAL IMPROVEMENT AND OTHER ONE-TIME FUNDING RECONCILIATION FY 2026 Per the Budget and Accountability Policy, the Office of Budget and Finance will reconcile and request the necessary budget adjustments for spending variances in the prior year forecasts each year by the end of October. Therefore, in accordance with A.R.S 42-17106(B), authorize the following amendments to the FY 2026 Annual Budget and 5 Year CIP Non Departmental (D470) Budget: 1. Adjust the FY 2026 General Fund (100) Non Recurring departmental budgets by the amounts indicated for the departments and appropriation unit groups listed under the General Fund Reconciliation section of the attached schedule. Offset these changes by a decrease to the Non Departmental (D470) General Fund (100) Non Recurring (NRNP) Contingency (4711) budget in the line “Unreserved Contingency” by $1,933,391. 2. Adjust the FY 2026 Detention Fund (255) Non Recurring departmental budgets by the amounts indicated for the departments and appropriation unit groups listed under the Detention Fund Reconciliation section of the attached schedule. Offset these changes by a decrease to the Non Departmental (D470) Detention Fund (255) Non Recurring (NRNP) Contingency (4711) budget in the line “Unreserved Contingency” by $1,790,033. 3. Adjust the FY 2026 Special Revenue Funds Non Recurring departmental budgets by the amounts indicated for the departments, funds, and appropriation unit groups listed under the Special Revenue Funds Reconciliation section of the attached schedule. Offset these changes by a decrease to the Non Departmental (D470) Non Departmental Grants (249) Non Recurring (NRNP) Contingency (4711) budget in the line “Unreserved Contingency” by $22,277. 4. Adjust the FY 2026 American Rescue Plan Act Funds Non Recurring departmental revenue and expenditure budgets by the amounts indicated for the departments, funds, and appropriation unit groups listed under the American Rescue Plan Act (ARPA) Fund Reconciliation section of the attached schedule. Offset these changes by an increase to expenditures and revenues for the Non Departmental (D470) Non Departmental Grants (249) Non Recurring (NRNP) budget by $3,205,796. 5. ACCEPTANCE OF 3RD YEAR GRANT FUNDS FROM ARIZONA GOVERNOR’S OFFICE FOR YOUTH, FAITH AND FAMILY, DISCRETIONARY FUND Approve the receipt of 3rd year grant funds from the State of Arizona, Governor’s Office of Youth, Faith, and Family (“GOYFF), Workforce Innovation Opportunity Act (“WIOA”) Grant Program –Discretionary Fund, Grant No. GR-WIOA-GOYFF-100123-08Y3 (CFDA No. 17.259) for performance period of October 1, 2025, through September 30, 2026. The 3rd year funding amount is $500,000 for the delivery of WIOA training activities. The purpose of the Grant is to develop and deploy innovative WIOA pilot programs that address gaps surrounding access to training that provide participants industry recognized credentials upon completion. The Program targets service delivery toward populations in Maricopa County most in need, namely Youth, Dislocated Worker, Veterans, those deficient in basic-skills and receiving public assistance, in the communities of Gila Bend, Wickenburg, Queen Creek, and closely surrounding areas. The purpose of the 3rd year grant is to continue training programs in the identified communities. Activities under this grant are administered by the Human Services Department Workforce Development Division Arizona@Work Maricopa County. The Program supports a training provider to deliver training in the identified areas. As a current recipient of the Arizona Governor’s Office for Youth, Faith and Family, Discretionary Fund Grant Program, the County received 3rd year funds through a non-competitive process. The grant funds are a recurring award. Receipt of the grant funds does not require future or ongoing contributions by the County at the end of the performance period. The Program will leverage existing WIOA training resources to support participants with wrap-around services for up to 12 months, post-completion of training. The County is not required to provide cash or in-kind match. The Human Services Department indirect rate of 24% by the U.S. Department of Health and Human Services for FY2026 is for salaries and employee related expenses. The 3rd year Grant amount is $500,000 of which no costs will be allocated for salaries and ERE. Indirect costs will be absorbed by the Human Services Department budget. The awarded grant funds will be passed through to training providers and are not subject to indirect cost recovery. The services provided under this grant are not a mandated function but provide a benefit to Maricopa County residents by providing career and training opportunities, assistance to job seekers, youth, dislocated workers, veterans, and to employers that need to fill vacancies. Program services help to establish a stronger workforce in the local area. The County contracts with Maricopa County Community College District to provide industry recognized training and credentials in the communities listed above. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore, expenditure of the revenues is not prohibited by the budget law. This Grant award does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall budget will be adjusted as necessary to accommodate the grant funding through a future reconciliation. Acceptance of the 3rd grant funds will not impact the County General Fund. APPLICATION TO U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES FOR A CHANGE IN SCOPE FOR THE HEAD START/EARLY HEAD START GRANT Approve the submittal of an application to the U.S. Department of Health and Human Services (DHHS)/Administration for Children and Families (ACF), Office of Head Start (OHS), Grant No. 09CH012079 for a modification to the current scope of work. The Maricopa County Head Start program will request a reduction to the grant’s enrollment of Early Head Start and Head Start number of children served. Also request authorization for the Chairman to sign the Application for Federal Assistance SF-424 form, to be submitted to OHS. This FY2026 Grant Award is non-competitive and awarded to the County because the County is a current Head Start program operator and recipient of Head Start funds. The services provided under this grant are not a mandated function but provide a benefit to the citizens by providing eligible children with high quality care and education by integrating Head Start performance measures. The current approved Early Head Start (EHS) enrollment is 368 infants, toddlers, and pregnant women, and Head Start (HS) enrollment is 715 for 3-to-5-year-olds. Through this action, the Human Services Department Early Education Division seeks to reduce enrollment for the current Budget period of FY2026 to 304 EHS and 571 HS. IGA WITH TOWN OF GUADALUPE FOR COMMUNITY DEVELOPMENT BLOCK GRANT ACTIVITIES Approve a financial Intergovernmental Agreement (“Agreement”) between the Town of Guadalupe (“Subrecipient”) and Maricopa County the (“County”) administered by its Human Services Department. The purpose of the Agreement is for the Subrecipient to remove and replace a total of 16 existing streetlights, and 18 new lights will be added, for a total of 34 streetlights to be installed. Mounting new LED fixtures (energy cost savings of roughly 50%) on higher poles and decreasing the distance between each pole will provide enhanced lighting for the neighborhood streets, thus improving neighborhood safety and security. The Subrecipient shall provide the services identified the Scope of Work. These activities meet CDBG program’s National Objectives as defined in 24 CFR § 570.208. IGA WITH CITY OF GLENDALE FOR SHELTER SERVICES Approve a financial Intergovernmental Agreement (Agreement) between the City of Glendale (“City”) and Maricopa County (“County”), administered by its Human Services Department. The City and the County are collectively referred to herein as the “Parties”. The purpose of this Agreement is to establish a partnership between the City and County to support the West Valley Housing Assistance Center (WVHAC), a subregional shelter that assists and houses unsheltered families who are located within West Valley cities. Regional city partners will work together to support utilizing and sustaining the WHAC for unsheltered families referred there. A subcontracted emergency shelter service provider, under an agreement with the County, is located at WVHAC and assists families and individuals with case management and support services to transition into housing. This agreement will give priority referral status at the WVHAC to Glendale, Peoria, and Surprise. The City shall provide the County with $100,000 for program activities. The Agreement is retroactive to and begins on July 1, 2025, and is effective through June 30, 2026. This agreement may be extended upon acceptance and approval by the Parties. This is a non-reoccurring Agreement that is entered by the Parties through a non-competitive process. Cash, match, or in-kind are not required. The County does not have any obligations after the Agreement term. The services provided under this Agreement are not mandated services but provide a benefit to the residents of Maricopa County by increasing the availability of shelter services to individuals and families experiencing homelessness. RECEIPT OF FUNDS FROM ARIZONA DEPARTMENT OF EDUCATION FOR THE CHILD AND ADULT CARE FOOD PROGRAM Approve the acceptance of funds from the Arizona Department of Education (ADE) Child and Adult Care Food Program (CACFP). CACFP funds are provided to Maricopa County and administered by the Human Services Department Early Education Division, Maricopa County Head Start Program. The Head Start Program submitted a funding application to ADE for $977,042 for Program Year 2025, the period of October 1, 2024, through September 30, 2025. Maricopa County Head Start program is an annual reoccurring recipient of CACFP funds from ADE with the submission of an application. Also request authorization for the Human Services Department Director to sign all documents related to acceptance of funds and any additional awards issued subsequently up to the estimated funding amount, as noted in the Permanent Agreement with ADE fully executed in October 26, 2013 (C-22-14-027-G-00) and as amended on August 20, 2025 (C-22-14-027-G-14). The Funds are utilized to provide meals and snacks that meet 1/3 to 2/3 of the daily nutritional needs of the Head Start program participants. The Head Start program submits an annual grant application to the Arizona Department of Education Child and Adult Care Food Program, which is funded and administered at the federal level by the Food and Nutrition Service, an agency of the United States Department of Agriculture (USDA). CACFP provides federal funds to nonresidential child and adult care facilities, emergency shelters, eligible after-school programs and family day care providers who serve nutritious meals and snacks. In Arizona, the Arizona Department of Education (ADE) directly administers CACFP. The goal of CACFP is to improve and maintain the health and nutritional status of children and adults in care settings while promoting the development of good eating habits. The Human Services Department indirect rate for FY2026, approved by the U.S. Department of Health and Human Services is 24% for salaries and employee related expenses. The awarded funds are $977,042 of which $0 is for salaries and ERE, therefore indirect costs are $0 and are not subject to indirect cost recovery. Annual Funding is reoccurring and does not require an in-kind or cash match or a commitment after the end of the term. The services provided by the funds are not mandated services but are a benefit to Head Start program participants by providing nutritious meals and snacks. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. The grant funds do not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. Pursuant to ARS 42-17106, approve the Year 3 Notice of Award of grant funds from Arizona Department of Health Services (AZDHS). Grant funds were awarded to AZDHS through Centers for Disease Control and Prevention's (CDC) Overdose Data to Action States. 2. The initial Intergovernmental Agreement between AZDHS and Maricopa County was approved under C-29-24-016-X-00, with a term of September 1, 2023 through August 31, 2028. The IGA includes an intended sub-recipient grant award allocation of $100,000 annually to Maricopa County Office of the Medical Examiner (OME) for toxicology testing in opioid and stimulant overdose deaths. This amendment authorizes acceptance of $100,000 during the grant award period 9/1/2025 to 8/31/2026. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation and, therefore, expenditures of these revenues are not prohibited by the budget law. The grant award may be awarded for a total of five years, with different award amounts each year. OME previously received $100,000 in FY26. The department has no in-kind match requirement or ongoing cash contributions during the grant award period. The grant award is not a mandated function but provides a benefit to the citizens by providing states and units of local governments with critical funding necessary to support a range of program areas including law enforcement, prosecution and court programs, prevention and education programs, corrections and community corrections, drug treatment and enforcement, crime victim and witness initiatives, and planning, evaluation, and technology improvement programs. The grant award is non-competitive. Any unallowable indirect costs will be absorbed by the departments. 3. AMENDMENT TO CONTRACT WITH NURSE FAMILY PARTNERSHIP D/B/A/ CHANGENT FOR IMPLEMENTATION OF THE NURSE-FAMILY PARTNERSHIP PROGRAM Approve Amendment 3 to the Contract between Nurse Family Partnership d/b/a/ Changent and Maricopa County by and through its Department of Public Health (MCDPH), Office of Family Health, for the implementation of the Nurse Family Partnership Program. MCDPH will exercise the automatic renewal clause and extend the expiration date to October 31, 2026. In addition, Exhibit D will be replaced with an updated Fees listing for the new year. An additional $60,000 for the new term shall be added to the agreement for a new not to exceed amount of $240,000. All other terms and conditions of the original contract shall remain in full force and effect. Fees for NFP training/education services will be billable and paid through grant funding from Arizona Department of Health Services (MIECHV). The grant award for the Nurse-Family Partnership Home Visitation in South Phoenix was approved by the Board on March 9, 2022, as C-86-22-124-X-00. Procurement authority for this expenditure is pursuant to MC1-102C, Prior Designation. This Agreement specifies the mechanisms and means for continued implementation of the Program and NFP’s training and support services. This Agreement allows the Office of Family Health to use the NFP model in its provision of community health nursing services. MCDPH, Office of Family Health, has adopted and implemented the Nurse Family Partnership model in the South Phoenix Region to provide comprehensive community health nursing services to eligible families. Through this program, MCDPH has established and expanded home visitation programs to support parents and caregivers in the ability to promote their children’s optimal development and health. PURCHASE ORDER FOR GRANT AWARD WITH ADHS FOR THE NURSE-FAMILY PARTNERSHIP FOR MIECHV, HOME VISITATION PROGRAM Approve Purchase Order (PO) MPO20260200 for Grant Award (RFGA2022-003-007) by the Arizona Department of Health Services (ADHS) to Maricopa County by and through its Department of Public Health (MCDPH) for the Nurse Family Partnership (NFP) to provide additional funding for the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Home Visiting Services Program. The PO was issued by ADHS on September 19, 2025. The total not-to-exceed amount is $470,614.61 for the period October 1, 2025, through September 30, 2026. The period of performance for the grant award is January 14, 2022, through January 13, 2027. The Department of Public Health FY26 indirect rate is 16.69%; therefore, indirect costs are estimated at $67,311.32 all of which are recoverable. Departmental indirect rates are re-established at the beginning of each fiscal year, and the future indirect rates will be collected at the corresponding rates. This competitive grant award is reoccurring and has been awarded to the department in previous years. There is no cash or in-kind match required. Should the grant cease, future ongoing cash contributions are not required. The grant award is not a mandated function but provides a benefit to Maricopa County by providing evidence-based home visiting models to communities identified to be at risk. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. NOTICE OF AWARD TO ACCEPT GRANT FUNDS FROM THE CENTERS FOR DISEASE CONTROL AND PREVENTION FOR EPIDEMIOLOGY AND LAB CAPACITY Approve the retroactive Notice of Award (NOA) 6 NU51CK000383-02-01and acceptance of grant funds from The Centers for Disease Control and Prevention (CDC) in the not-to-exceed amount of $722,226.00. This grant is to support Epidemiology and Laboratory Capacity for Prevention and Control of Emerging Infectious Diseases (ELC). This NOA was issued by the CDC on September 24, 2025. This is a partial NOA that brings the total funding for this budget period up to the not-to-exceed amount of $2,321,118.00. The supplemental award period began on August 01, 2025, and ends July 31, 2026. The original term of the grant began on August 01, 2024, and ends July 31, 2029. This grant deviates from County Policy A2505 and does not allow for full indirect cost reimbursement, but a maximum of 10%. The full indirect costs are estimated at $109,581.38 of which $65,656.91 is recoverable and $43,924.47 is unrecoverable. Program costs not covered by the grant will be subsidized by the MCDPH indirect cost pool. This grant is competitive, and it is unknown whether the grant will be awarded again. It should be noted that this grant award has not been awarded to Department of Public Health in previous years. There is no in-kind match requirement, and the grant does not require on-going cash contributions. The grant award is not a mandated function but provides a benefit to citizens by preventing and controlling emerging infectious diseases. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore, expenditure of the funds is not prohibited by the budget law. This Agreement shall become effective as of the date it has been executed by all PROJECT PARTNERS and shall expire ten years from that date. Background: The PROJECT is located within the City of Phoenix and unincorporated Maricopa County along 27th Avenue and Olney Avenue and is intended to mitigate excess stormwater runoff that impacts existing and proposed residences and businesses by using new storm drains and other drainage features along 27th Avenue and Olney Avenue. The proposed 27th Avenue and Olney Storm Drain Project is within the Laveen Area Drainage Master Study/Plan Update (Laveen ADMS/P U) study. The proposed project will be designed and constructed for a 10-year level of protection. The PROJECT will consist of construction of new storm drain pipes with inlets along 27th Avenue and along Olney which will discharge into the existing 27th Avenue and South Mountain Avenue basin. AMENDMENT TO IGA FOR PARADISE RIDGE DRAINAGE IMPROVEMENTS Approve Amendment No. 1 FCD IGA 2020A030A to Intergovernmental Agreement FCD 2020A030 for the Paradise Ridge Drainage Improvements Project (PROJECT) between the City of Phoenix (CITY), and the Flood Control District of Maricopa County (DISTRICT). This Agreement shall become effective as of the date it has been executed by all PROJECT PARTNERS and shall expire ten years from that date. The PROJECT is within the CITY’s jurisdiction and is bounded by State Route 101 on the south, Pinnacle Peak Road on the north, Scottsdale Road on the east, and the 64th Street alignment on the west. The land within the area described above is currently State Trust Land managed by the Arizona State Land Department. This PROJECT will mitigate the Rawhide Wash alluvial fan flooding hazard west of Scottsdale Road by capturing and conveying the 100-year flow in a new drainage channel and appurtenant structures along the approximate Rawhide Wash alignment, west of Scottsdale Road to the existing culvert system at State Route 101. The PROJECT is the next downstream segment of the Rawhide Wash drainage solution and was identified in the Pinnacle Peak West Area Drainage Master Study. Construction cost increases have caused an overall PROJECT cost increase, those cost increases being acceptable to the project partners. This Amendment No. 1 defines the increased PROJECT COST and the associated project partner cost share amounts with the cost share percentages remaining unchanged at 50% each. The current total PROJECT cost is now estimated to be $68,000,000 due to construction cost increases. This Agenda Item impacts Supervisorial District 3. (C-69-20-056-X-01)

AMENDMENT TO IGA FOR THE ROOSEVELT STREET STORM DRAIN PROJECT Approve Amendment No. 1 FCD IGA 2022A017A to Intergovernmental Agreement FCD 2022A017 for the Roosevelt St. Storm Drain Project (PROJECT) between the City of Tempe (TEMPE), the City of Scottsdale (SCOTTSDALE), and the Flood Control District of Maricopa County (DISTRICT). This Agreement shall become effective as of the date it has been executed by all PROJECT PARTNERS and shall expire ten years from that date. The PROJECT extends existing storm drain outlets to Indian Bend Wash at Roosevelt St., and includes new lateral storm drains to capture the flow in the east-west streets north of Roosevelt Rd./Continental Dr. New inlets will be added as well, with all these improvements intended to improve drainage conditions in areas south of Roosevelt St./Continental Dr. The PROJECT was recommended in the Lower Indian Bend Wash Area Drainage Master Study. In June 2022, TEMPE submitted the PROJECT for consideration in the DISTRICT’s Fiscal Year 2022/2023 Capital Improvements Program. TEMPE will be the lead agency for the PROJECT. Construction cost increases have caused an overall PROJECT COST increase, those cost increases being acceptable to the PROJECT PARTNERS. This Amendment No. 1 defines the increased PROJECT cost and the associated PROJECT PARTNER cost share amounts with the cost share percentages remaining unchanged. The estimated contract price is $500,000.00. The contract will be effective for 1,095 calendar days, from the date of the Notice to Proceed to Final Completion, with possible extensions. The Flood Control District of Maricopa County (District) and Consultant will mutually agree to a detailed scope of work for each work assignment. This is a qualifications-based selection in accordance with the Maricopa County Procurement Code, Article 5, Paragraph 504, and Article 5 Procurement Procedures Manual, Chapter II – Section 2.
