GRANTEE:  Maricopa County Department of Public Health
PURPOSE OF AMENDMENT: This amendment is to reflect additional investment for one ADE funded site resulting in the 
overall funding amount increasing.
THEREFORE, the Parties amend the Agreement as follows: 
1.
The total award amount for the grant period will increase from $2,403,440 to $2,407,390, a total increase of $3,950.

 Number of Slots:  (E. Maricopa – 99, NW Maricopa – 100, Phoenix North – 160, Phoenix South – 152, SE 
Maricopa   – 84, SW Maricopa – 27, QF Buy-In: E. Maricopa – 2, SW Maricopa – 1, ADE – 1).
