AMENDMENT TO CONTRACT

MARICOPA COUNTY
by and through
DEPARTMENT OF PUBLIC HEALTH
and
Solari, Inc.
I.
The above-named contract is hereby amended as specified below:
A. Section 1.0 CONTRACT TERM end date shall be updated to October 31, 2025.
B. Exhibit A, Pricing, Grand Total – The not to exceed amount shall be increased by $750,000
for a new not to exceed amount of $3,728,518.00.
I.
Exhibit A, Pricing, Project 1 – Promotores Bilingual Helpline
1. Project 1 shall be increased by $620,000 making the new not to exceed amount
$3,418,033.00 for this line item.
2. Project 1 shall be billed at $199,859.50 per month for the period through June 30,
2025, and at $155,000 per month for the period of July 1, 2025 – Oct 31, 2025.
II.
Exhibit A, Pricing, Project 2 – Community Services Heat Helpline
1. Project 2 shall be increased by $130,000 to be used for the 2025 Heat Season
making the new not to exceed amount $310,485 for this line item.
2. Project 2 is on a cost-reimbursement basis based on the number of units billed for
transportation.
C. Exhibit B, Scope of Work, Item 2.1 Project 1 – Promotores Bilingual Helpline
I.
Item 2.1.3, number of Bilingual Promotores is changed to 20 for the period July 1,
2025 – Oct 31, 2025.
D. Exhibit B, Scope of Work, Item 2.2 Project 2 – Community Services Heat Helpline
I.
Item 2.2.1 is amended to include a second performance period of May 1, 2025 –
October 31, 2025.
II.
All other terms and conditions of the original contract shall remain in full force and effect.
