Amendment No. One (1) to the 2024-2025
Independent Contractor Agreement
Contract No. 07012024-25

The Independent Contract Agreement dated July 1, 2024 between Arizona Community Action
Association (dba Wildfire) and City of Glendale- Community Action Program (hereinafter
“Contractor”) to conduct application intake and eligibility determination for utility bill assistance
and deposits is hereby amended as follows:

Amendment to:

Section I. Services and Programs — 1.2 Fund Sources:

1. APS: The Direct Service amount will increase from $27,475.00 to
$44,183.23. Program Delivery amount will increase from $5,495.00 to
$8,836.65 The total APS allocation amount will increase from $32,970.00
to $53,019.87.

Whole Agreement:

Unless otherwise noted herein, all other provisions of the original Agreement will remain in place
for the duration of the original Agreement.
