MARICOPA COUNTY, ARIZONA 
 
 
WHEREAS, Maricopa County (“County”) and Community Medical Services Holding, LLC (“CMS”) (“Contractor”) 
have entered into an Agreement to establish a program to increase access to Medication-Assisted Treatment (“MAT”), 
Opioid Use Disorder (“OUD”) recovery support services, and opioid prevention awareness for incarcerated patients 
transitioning to the community, executed March 1st, 2023 (“Agreement”).  
 
WHEREAS, County and Contractor have agreed to renew this Agreement. 
 
NOW, THEREFORE, the parties hereby agree to the following provision: 
 
The Agreement shall be renewed for a two-year term from March 1st, 2025, and ending February 28th, 2027.  
 
ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED. If there is any conflict between the 
terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both
