WHEREAS, Maricopa County (“County”) and Southwest Behavioral Health Services (“SWBHS”) (“Contractor”) 
have entered into an Agreement for the purpose of establishing a collaborative protocol executed on December 16, 
2022 (“Agreement”).  
 
WHEREAS, County and Contractor have agreed to renew this Agreement. 
 
NOW, THEREFORE, the parties hereby agree to the following provision: 
 
The Agreement shall be renewed for a term from December 16, 2024, and ending December 15, 2025.  
 
ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED. If there is any conflict between the 
terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both
