Contract No.: C-22-24-036-X-01 Amendment No. 1 Page 1 of 3 East Valley Institute of Technology (EVIT) AMENDMENT NO. 1 TO AFFILIATION AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND EAST VALLEY INSTITUTE OF TECHNOLOGY I. Maricopa County (“County”) administered by its Human Services Department (“Department”), and East Valley Institute of Technology District #401 (“EVIT”) fully executed a non-financial Affiliation Agreement (Agreement) on or about November 21, 2023. The term of the Agreement is for a three-year period from date of execution (November 21, 2023 through November 20, 2026.) The purpose of the Agreement is to provide internships and other hands-on training experiences for EVIT’s Program students (“Students”) as part of their overall education. EVIT and the County are collectively referred to as the “Parties” and individually as a “Party.” II. The Parties now agree to enter into this Amendment No. 1 to amend the Agreement as follows: A. Expand the educational internship programs as listed in attached Addendum to include the following in the Agreement: 1. Nursing Assistant 2. Dental Assistant 3. Medical Assistant 4. Practical Nursing 5. Mental and Social Health Technician 6. Emergency Medical Technician (EMT) B. Students will have the opportunity to complete their training experiences at all Maricopa County Head Start sites. C. EVIT shall provide the Program with lists of Students that will be participating in the internship programs. Students must have clear fingerprint clearance and background checks before entering the Head Start classrooms. D. The focus of Students’ training opportunities is to: 1. Observe children’s activities; 2. Interact with Head Start Teachers, Aides, and parents; 3. Shadowing staff; and 4. Participate in health, community, and school fairs, as well as other outreach activities. E. Revise 12. Add the following Sections to the Agreement: 26. Provisions Required By Law. Each and every provision of law and any clause required by law to be in this Agreement will be read and enforced as though it were included herein and, if through mistake or otherwise any such provision is not inserted, or is not correctly inserted, then upon the application of either party, this Agreement will promptly be physically amended to make such insertion or correction. Program Description 1. Early Childhood Education Program - designed to prepare individuals for employment in early childhood settings. This includes instruction in child growth and development, child health, nutrition, safety, planning, supervision of developmentally appropriate practices and learning activities, child guidance, family relationships, and parenting. 2. Nursing Assistant - prepares individuals to perform routine nursing-related services to patients in hospitals or long-term care facilities under the training and supervision of a registered nurse or licensed practical nurse. The program includes instruction in infection control, communication and interpersonal skills, safety and emergency procedures, personal care skills, and basic nursing skills. 3. Dental Assistant - prepares individuals to provide patient care, conduct dental radiographs (x-ray photographs), prepare patients and equipment for dental procedures, and discharge office administrative functions under the supervision of dentists and dental hygienists. 4. Medical Assistant - prepares individuals to provide medical office administrative services and perform clinical duties including patient intake and care, routine diagnostic and recording procedures, pre-examination and examination assistance, and the administration of medications and first aid. 5. Practical Nursing - prepares students to become Licensed Practical Nurses (LPNs) by providing instruction that combines theory with clinical practice. 6. Mental and Social Health Technician - an introduction to social work, mental health casework, clinical interviews, therapeutic intervention strategies, patient testing and evaluation procedures, patient and family counseling, social rehabilitation, patient care planning, record-keeping, and support services liaison activities. 7. Emergency Medical Technician (EMT) – prepares students to have the knowledge and skills needed to stabilize and safely transport patients using equipment typically found on an ambulance.
