RESOLUTION NO. 2627
A RESOLUTION OF THE MAYOR AND COUNCIL OF THE CITY OF TOLLESON
REGARDING NOTICE OF APPLICATION AND POTENTIAL CONFLICT OF INTEREST
UNDER THE CITY OF TOLLESON HOUSING REHABILITATION PROGRAM.
WHEREAS, the City of Tolleson is a participating jurisdiction in the United States
Department of Housing and Urban Development Community Development Block Grant (“CDBG”)
Program, which provides funding for the City of Tolleson Housing Rehabilitation Program; and
WHEREAS, the Housing Rehabilitation Program has received an application for services
from an individual who is a City of Tolleson appointed official; and
WHEREAS, the federal regulations governing the CDBG Program require that in order for
the applicant to be eligible for any benefit from the Housing Rehabilitation Program, the City of
Tolleson, as the participating jurisdiction, must provide disclosure of the potential conflicts
associated with this application; and
WHEREAS, the Mayor and Council deem it appropriate to provide the notice required by
the CDBG regulations.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND COUNCIL OF THE CITY OF
TOLLESON, ARIZONA, as follows:
Section 1. The recitals above are hereby incorporated as if fully set forth herein.
Section 2. Pursuant to the United States Government CDBG Program Conflict of Interest
Regulations (24 C.F.R. § 570.611), the City of Tolleson, as a participating jurisdiction in the CDBG
Program, hereby provides public notice of the potential conflict of interest raised by an
application by an individual to the Tolleson CDBG Program, in the form attached hereto as Exhibit
A.
Section 3. Said notice shall be posted by the City Clerk in a minimum of three conspicuous
locations in the City of Tolleson for 30 days following the adoption of this Resolution.
Section 4. The Mayor, City Manager, City Clerk and City Attorney are hereby authorized
and directed to take all steps necessary to carry out the purpose and intent of this Resolution.

NOTICE REGARDING POTENTIAL CONFLICT OF INTEREST RELATED TO THE CITY OF TOLLESON
HOUSING REHABILITATION PROGRAM
Pursuant to the United States Government Community Development Block Grant (“CDBG”)
Program Conflict of Interest Regulations (24 C.F.R. § 570.611), the City of Tolleson, as a
participating jurisdiction in the CDBG Program, hereby provides public notice of the potential
conflict of interest raised by an application by an individual to the Tolleson Housing Rehabilitation
Program.
The following individual has applied for assistance/aid under the City of Tolleson Housing
Rehabilitation Program, administered in partnership with the City of Tolleson by FSL Home
Improvements:
Diana L. Ruiz
The conflict of interest provisions of the regulations are triggered by the application of this
individual, as she is a City of Tolleson appointed official. Ms. Ruiz is a City of Tolleson Planning
and Zoning Commissioner.
The application of this individual for CDBG Program assistance falls under the Conflict of Interest
provisions of the CDBG Program regulations. The City of Tolleson provides this Notice pursuant
to those applicable regulations for purposes of requesting a permitted exception from the United
States Department of Housing and Urban Development to the conflict of interest restrictions, to
allow this applicant to receive the requested assistance from the City’s CDBG-funded Program.
