The Agreement originally effective February 1, 2024 and amended on March 13, 2024 is further 
amended as follows:
Change to Section 1:  Location of Practice:
Effective April 11, 2024, Contractor’s practice of medicine on behalf of the County shall be conducted at 
District 2 Medical Examiner’s Office, 560 Leonard Gray Way, Tallahassee Florida 32304, as from time to 
time amended.
All other terms and conditions of this contract are not affected by this Amendment and remain 
unchanged.
