INTERGOVERNMENTAL AGREEMENT

MARICOPA COUNTY 
By and Through
DEPARTMENT OF PUBLIC HEALTH
and
ARIZONA BOARD OF REGENTS ON BEHALF OF UNIVERSITY OF ARIZONA
for
IMMUNIZATION SERVICES 
I.
The above-named Intergovernmental Agreement (IGA) is hereby amended as specified below:
A.
Cover Page, Expiration Date is changed to May 31, 2024.
B.
Section IV. Compensation. Item 42. Deliverables and Delivery Schedule. to be added to the Contract.  The 
Contractor shall deliver the following items to MCDPH within specified and/or reasonable time frames:    
i.
Allow MCDPH access to the Teamup site that has the schedule of events and ensure the calendar is 
updated to allow MCDPH to refer clients in need of services offered by the Contractor.  
ii.
Provide monthly, number of health screenings, medical testings, outreach services, and immunizations by 
type of vaccine with invoice by the 25th of each month.
iii.
Screen all immunization clients for insurance to ensure use of appropriate vaccine, vaccine for 
adults/purchased vaccine and return immunization consent forms within 48 hours of an event.   
C.
Exhibit B-updated to show the end date as May 31, 2024, and to state Contractor obtained possession of the van on 
September 30, 2023.  
II.
All other terms and conditions of the original IGA and previous amendments shall remain in full force and effect.
