59. AGREEMENT WITH HUMAN SERVICES CAMPUS Approve an agreement between Human Services Campus (HSC) and Maricopa County to identify and address Housing Navigation. HSC and Correctional Health Services (CHS) shall operate in a professional manner that conforms to all local, state, and federal codes and rules and within the standard of practice for the scope of each of the parties’ responsibilities. HSC and CHS shall comply with all federal, state, and local laws, ordinances, rules, and regulations applicable to each party’s performance under this Agreement. There are no financial responsibilities assigned by this Agreement; the Agreement is intended to allow HSC and CHS to work together to better address the needs of the community. This Agreement shall begin upon approval by the Maricopa County Board of Supervisors and shall terminate one year thereafter unless otherwise terminated or amended as provided herein. By mutual written amendment executed by the Parties’ authorized representatives, this Agreement may be extended for supplemental periods of 12 months, up to maximum of 48 months. This agreement is effective July 26, 2023 until July 25,2024. (C-26-24-001-X-00)

31. AMENDMENT TO AGREEMENT WITH HUMAN SERVICES CAMPUS Approve Amendment No. 1 to the agreement between the Human Services Campus (HSC) and Maricopa County. This agreement intends to identify and address access to Housing Navigation resources for incarcerated patients transitioning into the community. HSC and Correctional Health Services (CHS) collaborate to provide linkages to housing resources to reduce homelessness. The purpose of this amendment is to renew for a term of twelve (12) months. It is non-financial and is effective July 26, 2023, and ending July 25, 2024. In addition, replace “Human Services Campus” with “Human Services Campus, Inc. dba Keys to Change” to rectify the recent name change. (C-26-24-001-X-01)

Rescinds and replace Amendment no. 1 to agreement with Human Services Campus and Maricopa County. The dates of term were incorrectly stated as from July 26, 2023, and ending July 25, 2024. 37. AMENDMENT TO AGREEMENT WITH HUMAN SERVICES CAMPUS, INC. DBA KEYS TO CHANGE Approve Amendment No. 2 to the agreement between Human Services Campus, Inc. (HSC) dba Keys to Change and Maricopa County through the Department of Correctional Health Services (CHS). This agreement establishes collaborative protocols for HSC and CHS to link incarcerated patients to housing resources in the community upon release, to reduce homelessness.
