49. AMENDMENT TO ARIZONA DEPARTMENT OF HEALTH SERVICES IGA FOR WOMEN, INFANT AND CHILDREN SERVICES Approve Amendment 8 to Intergovernmental Agreement (IGA) Contract No. CTR040876 between Arizona Department of Health Services (ADHS), Women, Infant and Children (WIC) Services and Maricopa County by and through its Department of Public Health (MCDPH) to provide continued funding for WIC Services, Breast Feeding Peer Counseling Services Program (BFPC), WIC Special Projects, and LA Project. The Amendment adds LA Project to the WIC Services with funding of $62,524. In addition, the Amendment decreases the WIC Special Project funds by $91,976 for a total reduction of $29,452 for the combined total revised Price Sheet in the not-to-exceed amount of $10,286,186. The budget period begins October 1, 2021 through September 30, 2022. The IGA term is from October 1, 2018 through September 30, 2023. It is mutually agreed that the Intergovernmental Agreement referenced is amended as follows: 1. The Scope of Work is revised and replaced 2. The Price Sheet is revised and replaced 3. Exhibit C is revised and replaced; and 4. Exhibit D has been added All other terms & conditions remain in their entirety. Maricopa County Department of Public Health’s Indirect Rate for FY22 is 17.97%. Indirect costs are estimated at $1,439,489 for WIC, $80,561 for BFPCFD & $244,792 for WICSPROJ & $9,524 for WICLAP. For a total of $1,774,366 total indirect. The WIC Services award is reoccurring and has been awarded to the department for many years. This grant does not require match, indirect costs are fully recoverable and ongoing cash contributions are not required. The grant award is not a mandated function but provides a benefit to the citizens by providing vouchers for approved foods (milk, cheese, cereals, peanut butter, eggs, infant formula, and juice etc.) for low-income pregnant, post-partum, breast-feeding women, infants, and children up to five (5) years of age. Clients also receive nutrition and breast-feeding education and referrals to applicable social services. 61. AMENDMENT TO ARIZONA DEPARTMENT OF HEALTH SERVICES IGA FOR WOMEN, INFANT, AND CHILDREN SERVICES Approve Amendment 9 to Intergovernmental Agreement (IGA) Contract no. CTR04876 between Arizona Department of Health Services (ADHS), Women Infant and Children (WIC) Services and Maricopa County, by and through its Department of Public Health (MCDPH), to provide continued funding for WIC Services, Breast Feeding Peer Counseling Services Program (BFPC), WIC Special Projects, and LA Project. This Amendment adds additional LA Project Funding to the WIC Services with funding of $194,651 for budget period April 1, 2022 to September 30, 2022. In addition, the Amendment increases the BPCS Project funds by $18,550 for a total increase of $213,201 for Amendment 9. The budget period begins October 1, 2021 through September 30, 2022. CTR04876 between Arizona Department of Health Services (ADHS), Women Infant and Children (WIC) Services and Maricopa County, by and through its Department of Public Health (MCDPH), to provide continued funding for WIC Services, Breast Feeding Peer Counseling Services Program (BFPC), WIC Special Projects. Additional monies have been added to Other Operating Expenses (Local Agency Projects Funding) of $252,066 for Maricopa County for the Hope Center WIC Clinic site that will include costs related to the clinic set up including furniture, cubicle and equipment installation; computer, printer, and internet installation; and data and electrical infrastructure costs. This Amendment has a combined total Price Sheet in the not-to-exceed amount of $10,139,664.00 (WIC - $9,099,160, BFPC - $570,000, WIC Special Projects - $218,438 and Local Agency Projects Funding-$252,066.00). The budget period covers October 1, 2022 through September 30, 2023. The IGA term is from October 1, 2018 through September 30, 2023.
