AMENDMENT TO REQUEST FOR PROPOSAL CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve an amendment to Request for Proposal (RFP) Contract 190140 between the Delta Dental Plan of Arizona (DDPA) and Maricopa County through its Department of Public Health (MCDPH). This amendment shall extend the Expiration Date to February 28, 2023. In addition, the Annual Maximum Benefits has been updated. From October 1, 2020, thru February 28, 2022, the maximum rate increased from $1,500 to $5,000 per person per benefit year. From March 1, 2022, thru February 28, 2023, the maximum rate shall be decreased from $5,000.00 to $2,500.00 per person per benefit year. The purpose of this Amendment is to continue providing Oral Health Services (Provision of Dental Insurance Premiums) according to the most current CDC guidance and recommendations to person infected with HIV/AIDS. These services will be funded with grant funds received through the Ryan White HIV/AIDS Treatment Extension Act of 2009. FUNDING AUTHORITY INCREASE FOR REQUEST FOR PROPOSAL CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve a funding authority increase in the amount of $3,000,000 for Request for Proposal (RFP) Contract 190140 between the Delta Dental Plan of Arizona (DDPA) and Maricopa County through its Department of Public Health (MCDPH). This increase shall be used for services rendered in the current budget period beginning 3/1/2022 thru 2/28/2023 and it shall bring the total not-to-exceed amount from $6,000,000 to $9,000,000 over the life of the contract that began 3/1/2019. This amendment shall extend the Expiration Date to February 28, 2024. The Annual Maximum Benefits from March 1, 2023, through February 28, 2024, shall remain at $2,500.00 per person per benefit year. The purpose of this Amendment is to continue providing Oral Health Services (Provision of Dental Insurance Premiums) according to the most current CDC guidance and recommendations to person infected with HIV/AIDS. These services wi ll be funded with grant funds received through the Ryan White HIV/AIDS Treatment Extension Act of 2009. The maximum amount of reimbursement of this contract is not to exceed $2,000,000 over the next year. Funding for this Contr act is provided by a grant from the Federal Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County General Govern ment Department, Ryan White Part A Program and will not affect the County general fund. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the b udget law. This Contract does not alter the budget constraining expenditures of the local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future re conciliation. (C-86-19-025-3-04)

AMENDMENT TO CONTRACT PH RFP 190140 BETWEEN MARICOPA COUNTY BY AND THROUGH THE DEPARTMENT OF PUBLIC HEALTH AND DELTA DENTAL PLAN OF ARIZONA Approve Amendment 6 to PH RFP 190140 between Maricopa County by and through the Department of Public Health (MCDPH) and Delta Dental Plan of Arizona. This amendment shall revise the language in Exhibit B, Section 3.0 as shown below: A. Exhibit B Section 3.0 – Applicant Eligibility language shall be revised as shown below: From: “The dental insurance plan under this RFP is a payer of last resort; therefore, enrolled individuals cannot be enrolled in another dental insurance plan and be eligible to receive benefits under this RFP.” To: “Individuals enrolled in the dental insurance plan under this RFP are allowed to be enrolled in another dental insurance plan. However, the dental insurance plan under this RFP is determined to be the payor of last resort and will therefore be deemed secondary payor. Under the coordination of benefits provisions, this plan will always be the secondary policy, and claims paid will not exceed total claim charges.” All other terms and conditions shall remain in full force and effect.
