81. AMENDMENT TO CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Due to COVID-19, the Ryan White Planning Council voted to decrease the ‘Plan Year Benefit Maximums’ in the provision of dental insurance services to individuals affected by HIV/AIDS. Approve amendment 1 to Request for Proposal (RFP) Contract 190140 between the Delta Dental Plan of Arizona (DDPA) and Maricopa County through its Department of Public Health (MCDPH). This amendment decreases the ‘Plan Year Benefit Maximum’ from $3500 per person per benefit year, to $1500 per person per benefit year. The decrease is retro-active to May 1, 2020. During this time of pandemic, the Ryan White Program has identified more urgent needs for this funding. At the Ryan White Program’s request, the contract amount will remain in the not-to-exceed amount of $6,000,000. The Delta Dental budget is currently under review and if data warrants, the budget will be adjusted. The term for this Contract is 03/01/2019 through 02/29/2022. Funding for this Contract is provided by a grant from the Federal Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County General Government Department, Ryan White Part A Program and will not affect the County general fund. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Contract does not alter the budget constraining expenditures of the local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-19-025-3-01)

AMENDMENT TO REQUEST FOR PROPOSAL CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve an amendment to Request for Proposal (RFP) Contract 190140 between the Delta Dental Plan of Arizona (DDPA) and Maricopa County through its Department of Public Health (MCDPH). This amendment shall extend the Expiration Date to February 28, 2023. In addition, the Annual Maximum Benefits has been updated. AMENDMENT TO CONTRACT PH RFP 190140 BETWEEN MARICOPA COUNTY BY AND THROUGH THE DEPARTMENT OF PUBLIC HEALTH AND DELTA DENTAL PLAN OF ARIZONA Approve Amendment 6 to PH RFP 190140 between Maricopa County by and through the Department of Public Health (MCDPH) and Delta Dental Plan of Arizona. This amendment shall revise the language in Exhibit B, Section 3.0 as shown below: A. Exhibit B Section 3.0 – Applicant Eligibility language shall be revised as shown below: From: “The dental insurance plan under this RFP is a payer of last resort; therefore, enrolled individuals cannot be enrolled in another dental insurance plan and be eligible to receive benefits under this RFP.” To: “Individuals enrolled in the dental insurance plan under this RFP are allowed to be enrolled in another dental insurance plan. However, the dental insurance plan under this RFP is determined to be the payor of last resort and will therefore be deemed secondary payor. Under the coordination of benefits provisions, this plan will always be the secondary policy, and claims paid will not exceed total claim charges.” All other terms and conditions shall remain in full force and effect. This amendment shall revise the language in Appendix A, Summary of Benefits, to reflect the rates in effect regarding the Plan Year Benefits Maximum per person as follows: From 03/01/2024 – 07/31/2024, the maximum rate shall remain $2,500 per person per benefit year. From 08/01/2024 – 02/28/2025, the maximum rate shall be decreased to $1,500 per person per benefit year. All other terms and conditions shall remain in full force and effect. (C-86-19-025-3-07)
