AMENDMENT TO RFP CONTRACT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HIGH-RISK PERINATAL PROGRAM/NEWBORN INTENSIVE CARE PROGRAM COMMUNITY HEALTH NURSING PROGRAM Approve Amendment 4 to the ADHS Contract for Community Nursing Services CTR046941 awarded by the Arizona Department of Health Services (ADHS) High Risk Perinatal Program (HRPP) with Maricopa County by and through its Department of Public Health (MCDPH). The funding for this program is “as needed” based on client visits/enrollments and is estimated at a not-to-exceed amount of $590,000 for the budget period August 1, 2022 through June 30, 2023 (Year 4). Payment on an as needed basis allows ADHS greater flexibility in allocating funds based on the services being provided. The contract term is from August 1, 2019 through June 30, 2023. The Department of Public Health indirect rate for FY22 is 17.97%. The indirect costs are estimated at $89,872.85 and are fully recoverable. The HRPP NICP grant award is reoccurring and has been awarded to the department in prior years. This competitive grant does not require an in-kind match, indirect cost is fully recoverable, and ongoing cash contributions are not required. The grant award is not a mandated function but provides a benefit to the citizens by providing developmental follow-up through home-based services for families whose children received intensive or special care immediately after birth and for children with identified special health care needs. All program costs are allocated to the grant so there will be no additional burden on the department’s operating budget. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation and therefore expenditure of the revenues is not prohibited by the budget law. This action does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this grant will be provided by ADHS and will not affect the County’s general fund. (C-86-20-008-3-08)

AMENDMENT TO RFP CONTRACT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES (ADHS) FOR HIGH-RISK PERINATAL PROGRAM/NEWBORN INTENSIVE CARE PROGRAM COMMUNITY HEALTH NURSING PROGRAM Approve Amendment 5 to the ADHS Contract for Community Nursing Services CTR046941 awarded by the Arizona Department of Health Services (ADHS) High Risk Perinatal Program (HRPP) with Maricopa County by and through its Department of Public Health (MCDPH). Previously the BOS approved receiving a potential $590,000 for the budget period July 30, 2022, through June 30, 2023; however, ADHS has increased the maximum available funding to a total not to exceed amount of $603,250. Therefore, MCDPH requests this additional $13,250 to be approved if needed. The funding for this program is on an “as needed basis” based on client visits/enrollments. Payments on an as needed basis allows ADHS greater flexibility in allocating funds based on the services being provided. The original contract term is from August 1, 2019 through June 30, 2023 and we are in Year 4. The Department of Public Health indirect rate for FY23 is %17.24. The indirect costs are estimated at $1,948.40 and are fully recoverable. The HRPP NICP grant award is reoccurring and has been awarded to the department in prior years. This competitive grant does not require an in-kind match, indirect cost is fully recoverable, and ongoing cash contributions are not required. The grant award is not a mandated function but provides a benefit to the citizens by providing developmental follow-up through home-based services for families whose children received intensive or special care immediately after birth and for children with identified special health care needs. The funding for the program is based on client visits/enrollments and is in the not-to-exceed amount of $483,330.15 for the budget period July 01, 2023 through June 30, 2024. The contract term is also from July 01, 2023 through June 30, 2024. The Department of Public Health indirect rate for FY24 is 15.65%. The indirect costs are estimated at $65,405.25 and are fully recoverable. The HRPP NICP grant award is reoccurring and has been awarded to the department in prior years. This competitive grant does not require an in-kind match, indirect cost is fully recoverable, and ongoing cash contributions are not required. The grant award is not a mandated function but provides a benefit to the citizens by providing developmental follow-up through home-based services for families whose children received intensive or special care immediately after birth and for children with identified special health care needs. All program costs are allocated to the grant so there will be no additional burden on the department’s operating budget. AMEND IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR THE HRPP COMMUNITY HEALTH NURSING SERVICES Approve Amendment 6 by and through the Department of Public Health and the Arizona Department of Health Service for the High Risk Pregnancy Program (HRPP) Community Health Nursing Services. The above-named contracts are hereby amended as specified below: 1. Pursuant to the Special Terms and Conditions, Provision Three (3), Contract Extension (6-Year Maximum), the Contract is hereby extended through August 31, 2024. 2. Pursuant to the Uniform Terms and Conditions, Provision Five (5), subsection 5.1 Amendments, the Special Terms and Conditions are revised and replaced: 2.1.
