AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF UNIVERSITY OF ARIZONA Approve Amendment 1 to Intergovernmental Agreement (IGA) between Arizona Board of Regents for and on behalf of University of Arizona (U of A) and Maricopa County by and through its Department of Public Health (MCDPH) to include influenza vaccinations to be administered in addition to the current Covid vaccinations currently being administered. Amendment 1 shall delete the current Purpose in Section III, Work Statement and replace it with a revised version. In addition, the current Compensation section shall be deleted in its entirety and replaced with a revised version that allows for changes in the reimbursement rates to be communicated more efficiently by directing Contractors to a MCDPH website. Finally, the Expiration date shall be extended through June 30, 2022. There are no changes in the Agreement Amount. (C-86-21-147-X-01)

AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF UNIVERSITY OF ARIZONA Approve Amendment 2 to Intergovernmental Agreement (IGA) between Arizona Bo ard of Regents for and on behalf of University of Arizona (U of A) and Maricopa County by and through its Department of Public Health (MCDPH). The agreement provides funding for U of A Mel and Enid Zuckerman College of Public Health (MEZCOPH) for Covid-19 & Influenza mobile vaccination services in order to increase access for hard-to-reach populations. AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF UNIVERSITY OF ARIZONA Approve Amendment 3 to Intergovernmental Agreement (IGA) between Arizona Board of Regents for and on behalf of University of Arizona (U of A) and Maricopa County by and through its Department of Public Health (MCDPH). The agreement provides funding for U of A Mel and Enid Zuckerman College of Public Health (MEZCOPH) to include a mobile vaccination clinic which will provide vaccination services for individuals in hard-to-reach populations. I. The above-named Intergovernmental Agreement (IGA) is hereby amended as specified below: A. Cover Page, Agreement Amount is changed from $250,000 to $806,364. The additional $556,364 is to be used for a new Scope of Work (Exhibit B) beginning on Page 2 of this Amendment. B. Cover Page, Expiration Date is changed to September 30, 2023. C. New Exhibit B - Mobile Vaccine Clinic - SW Maricopa County Outreach and Engagement has been added II. Compensation has been updated and replaced beginning on Page 6 of this Amendment. III. Invoices has been added beginning on Page 6 of this Amendment. IV. All other terms and conditions of the original IGA and previous amendments shall remain in full force and effect. Funding for this amendment is provided by the Centers of Disease Control (CDC) Health Disparities Grant. (C-86-21-147-X-03)

AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS ON BEHALF OF UNIVERSITY OF ARIZONA FOR IMMUNIZATION SERVICES Approve Amendment 5 by and through the Maricopa County Department of Public Health (MCDPH) and the Arizona Board of Regents on Behalf of University of Arizona (U of A) for the Immunization Services. I. The above-named Intergovernmental Agreement (IGA) is hereby amended as specified below: A. Cover Page, Expiration Date is changed May 31,2024 to June 30, 2025. B. Section IV, COMPENSATION—Remaining funds will be invoiced monthly with all invoices submitted by July 31, 2025. II.
