43. CONTRACTS FOR BUILDING HEALTHY AND RESILIENT COMMUNITIES IN THE NORTHWEST REGION OF MARICOPA COUNTY Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more Contracts to be developed under #PH RFP 220203, Building Healthy and Resilient Communities - Northwest Region. The funding will be used to help develop and work through strategies and interventions that consider systemic barriers and potentially discriminatory practices. Funding must support Policy, System and Environmental changes (PSE) that can impact the disparity gap for vulnerable and underserved populations. Funds are not intended for direct services or direct financial distribution to the public. Funds are intended to assist populations instead of individuals. This solicitation (PH RFP 220203) is intended for residents in the Northwest Region which identified mental health issues, obesity, and high blood pressure or high cholesterol as priority areas from the completed Community Health Needs Assessment in July 2020 and successful respondents are required to address at least one of these priorities. The selected Contractors will be determined qualified to provide services by the Public Health Department’s Evaluation Committee. The resulting Contracts are granted a term of fourteen (14) months, beginning April 1, 2022 through May 31, 2023, with an option to renew for one-year terms up to a maximum of four (4) additional years (or at the County’s sole discretion, extend the contract on a month-to-month bases for a maximum of six (6) months after expiration) based on available funding and successful Contractor performance. The total amount to be spent on all awarded contracts under this solicitation is expected to be $2,500,000 over the initial term (first project period) with no Contractor receiving more than $500,000. This agreement will be funded by the COVID-19 Health Improvement Plan- Disparities Grant. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the contract. 43. AMENDMENTS TO CONTRACTS WITH BANNER HEALTH FOUNDATION, BENEVILLA, GLENDALE STRONG FAMILY NETWORK, AND YWCA FOR THE PURCHASE OF SERVICES RELATED TO BUILDING HEALTHY AND RESILIENT COMMUNITIES-NORTHWEST REGION INITIATIVE FOR MARICOPA COUNTY Approve Amendment(s) 1 by and through the Department of Public Health and Sub-Recipients Banner Health Foundation, Benevilla , Glendale Strong Family Network, and YWCA for the purchase of services related to building healthy and resilient communities-Northwest region initiative for Maricopa County. The above-named contracts are hereby amended as specified below: A. Section 6.0 Terms and Conditions shall be updated to include the following additional clauses: i . 6.45 WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-393.01: If Contractor engages in for-profit activity and has 10 or more employees, and if this agreement has a value of $100,000 or more, Contractor certifies it is not currently engaged in, and agrees for the duration of this agreement to not engage in, a boycott of goods or services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. ii. 6.46 WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-394: Contractor warrants and certifies that it does not currently, and agrees for the duration of the Agreement, that it will not use: 1. the forced labor of ethnic Uyghurs in the People's Republic of China, 2. any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China, 3. any contractors, subcontractors or suppliers that use the forced labor or any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China. If Contractor becomes aware during the term of this Agreement that the Contractor is not in compliance with this paragraph, Contractor shall notify the County within five business days after becoming aware of the noncompliance. If the Contractor fails to provide a written certification to the County that the Contractor has remedied the noncompliance within 180 days after notifying the County of its noncompliance, then the agreement terminates, except that if the agreement termination date occurs before the end of the 180-day period, the agreement terminates on the agreement termination date. B. Contract Expiration date for all contracts shall be updated to May 31, 2024. C. Scope of Work shall be revised for the following subrecipient contracts: i . Banner Health Foundation: No revisions to scope of work, however, compensation structure will change. ii. Benevilla : MCDPH will provide additional funding of $175,905.38 to fund the following activities/line items for one more year as detailed in the original contract: 1. Personnel ($73,692); 2. Fringe ($7,369.20); 3. Advertising Costs ($60,000); 4. CAB Attendance ($12,000); 5. Indirect ($22,944.18). iii. Glendale Strong: MCDPH will provide additional funding of $41,860 to fund the following activities/line items for one more year as detailed in the original contract: 1. Social Media Strategist ($18,200); 2. Non-profit Development Consultant ($18,200); 3. Indirect ($5,640) iv. YWCA: MCDPH will provide additional funding of $448,109.00 to fund the following activities/line items for one more year as detailed in the original contract: 1. Personnel ($262,000); 2. Fringe ($47,160); 3. Supplies ($5,000); 4. Computer and Accessories ($12,000); 5. Subscriptions ($6,000); 6. Printing and Design ($7,500); 7. Advertisement ($5,000); 8. Outreach ($35,000); 9. Translation Services ($10,000); 10. Indirect ($58,449). All other terms and conditions of all of the original contracts shall remain in full force and effect. (C-86-22-093-X-01)

65. AMENDMENT TO CONTRACT WITH BENEVILLA AND ARIZONA YWCA METROPOLITAN PHOENIX Approve Amendment 2 to PH RFP 220203 between Maricopa County by and through the Department of Public Health and Benevilla, and Arizona YWCA Metropolitan Phoenix. The amendment for Benevilla shall update the term expiration date to June 30, 2024, to fully expend funds and report on deliverables as well as all invoices must be received by July 31, 2024. The amendment for Arizona YWCA Metropolitan Phoenix shall update the term expiration date to December 31, 2024, and includes a new disbursement matrix shown on page two (2) of the amendment.
