48. CONTRACTS FOR BUILDING HEALTHY AND RESILIENT COMMUNITIES COUNTYWIDE FOR MARICOPA COUNTY Pursuant to MC1-329 of the Maricopa County Procurement Code, authorize the Chairman of the Board of Supervisors to execute one or more Contracts to be developed under #PH RFP 220204, Building Healthy and Resilient Communities – County-wide. The contracts aim to improve access to care and address health disparities. Funds are not intended for direct services or direct financial distribution to the public. Funds are intended to assist populations instead of individuals. This solicitation (PH RFP 220204) is intended for residents across all five regions of Maricopa County to increase access to care based on the completed Community Health Needs Assessment. The selected Contractors will be determined qualified to provide services by the Public Health Department’s Evaluation Committee. The chosen Contracts are granted a term of fourteen (14) months, beginning April 1, 2022 through May 31, 2023, with an option to renew for one-year terms up to a maximum of four (4) additional years (or at the County’s sole discretion, extend the contract on a month-to-month bases for a maximum of six (6) months after expiration) based on available funding and successful Contractor performance. At this time the total amount of funding is not expected to exceed $1,500,000 among all awarded Contractors for the first project period, however there is a chance that additional funding may become available. This agreement will be funded by the COVID-19 Health Improvement Plan- Disparities Grant. Should the funding source cease and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds, will apply allowing for a change or termination of the contract. AMENDMENT TO SUBRECIPIENT CONTRACT FOR VACCINE EQUITY WITH ARI ZONA DEPARTMENT OF HEALTH SERVICES Approve amendment No. 1 to Subrecipient Contract with The Arizona Partnership for Immunization (TAPI) to clarify the funding for the purchases of services related to immunization equity and health disparities. The base agreement was signed by the Board of Supervisors as C-86-22-100-X-00 on April 26, 2022 when TAPI was awarded under Public Health Request for Proposal No. 220204 for Building Healthy and Resilient Communities (County-wide) Initiative utilizing funding received from the Centers for Disease Control (CDC) Health Disparities' grant to address health inequities. The decision was later made to change the funding source of this particular proposal to be funded by our Arizona Department of Hea lth Service's (ADHS) Immunization Services grant as the proposed scope of work is in alignment with this grant's deliverables and in doing so, an additional award could be made to another community-based organization utilizing the Health Disparities' fundi ng. The Immunization Services grant is an Intergovernmental Agreement (IGA) ADHS18-177694, Immunization Services between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), Disease Contro l Division. The term of the agreement is 14 months, beginning on April 1, 2022 and ending May 31, 2023. This remains unchanged from the base agreement. The amount awarded to TAPI in the base agreement is $109,129.25 and this amendment clarifies this not-t o-exceed amount will come from the Immunization Services Grant. (C-86-22-100-X-01)

The original solicitation (PH RFP 220204) stated a maximum amount of $ 1,500,000 would be allocated among all successful respondents and the Board of Supervisors authorized that amount on January 26, 2022 as C-86-22-100-X-00 (item #48). This request is to increase the maximum threshold for this service as MCDPH received more responses than anticipated and would like to fund additional contractors from this opportunity. AMENDMENTS TO CONTRACTS WITH ASIAN PACIFIC COMMUNITY IN ACTION, AZ CHAPTER OF AMERICAN ACADEMY OF PEDIATRICS, BOYS HOPE GIRLS HOPE OF AZ, COMMUNITY ALLIANCE, HUSHABY NURSERY, NOTMYKID, PHOENIX MIDWIFE AND VALLEY OF THE SUN YMCA Approve Amendment(s) by and through the Department of Public Health and Sub-Recipient contracts for Asian Pacific Community in Action, AZ Chapter of American Academy of Pediatrics, Boys Hope Girls Hope of AZ, Community Alliance, Hushabye Nursery notMykid , Phoenix Midwife and Valley of the Sun YMCA for the purchase of services related to building healthy and resilient communities-countywide initiative for Maricopa County. The above-named contracts are hereby amended as specified below: A. Section 6.0 Terms and Conditions shall be updated to include the following additional clauses: i . 6.45 WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-393.01: If Contractor engages in for-profit activity and has 10 or more employees, and if this agreement has a value of $100,000 or more, Contractor certifies it is not currently engaged in, and agrees for the duration of this agreement to not engage in, a boycott of goods or services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. ii. 6.46 WRITTEN CERTIFICATION PURSUANT to A.R.S. § 35-394: Contractor warrants and certifies that it does not currently, and agrees for the duration of the Agreement, that it will not use: 1. the forced labor of ethnic Uyghurs in the People's Republic of China, 2. any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China, 3. any contractors, subcontractors or suppliers that use the forced labor or any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China. If Contractor becomes aware during the term of this Agreement that the Contractor is not in compliance with this paragraph, Contractor shall notify the County within five business days after becoming aware of the noncompliance. If the Contractor fails to provide a written certification to the County that the Contractor has remedied the noncompliance within 180 days after notifying the County of its noncompliance, then the agreement terminates, except that if the agreement termination date occurs before the end of the 180-day period, the agreement terminates on the agreement termination date. B. Contract Expiration date for all contracts shall be updated to May 31, 2024. C. Scope of Work shall be revised for the following subrecipient contracts: i . Amendment 1: Asian Pacific Community in Action: MCDPH will provide additional funding of $63,827.67 to fund the following activities/line items for one more year as detailed in the original contract: 1. AMENDMENTS TO CONTRACTS WITH HUSHABYE NURSERY, ASIAN PACIFIC COMMUNITY IN ACTION, AND VALLEY OF THE SUN YMCA Approve Amendments to PH RFP 220204 contracts between Maricopa County by and through the Department of Public Health and Hushabye Nursery, Asian Pacific Community in Action, and Valley of the Sun YMCA. This second amendment for Hushabye Nursery shall update the term expiration date to August 31, 2024. This second amendment for Asian Pacific Community in Action shall update the term expiration date to December 31, 2024. Amendment 3 for Valley of the Sun YMCA shall update the term expiration date to July 31, 2024. All amendments include a new disbursement matrix shown on page two (2) of the amendment.
