99. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES (ADHS) FOR COVID-19 VACCINATIONS Approve a cost reimbursement Intergovernmental Agreement (IGA), CTR062177, between ADHS and Maricopa County by and through its Department of Public Health (MCDPH), Office of Community Wellness and Health Promotion, to provide funding for the COVID-19 Vaccinations. This grant will assist MCDPH to continue to enhance program stewardship and accountability for all publicly purchased vaccine in support of the Vaccines for Children Program (VFC) and the Vaccines for Adults Program (VAP). It will also provide vaccines to children and adults in accordance with recommendations of the CDC Advisory Committee on Immunization Practices (ACIP) and prevent and control Vaccine-Preventable Diseases (VPD). This is a new agreement however it is funded by COVID-19 grants. Additionally, the purpose of this IGA is to separate COVID-19 Immunization fund from non-COVID-19 immunization funds. The term of this Agreement is July 1, 2022, through termination date, June 30, 2024. The full amount as listed on this new agreement is in a not to exceed of $28,346,200. Nonetheless, $4,256,944.22 has been spent in FY21 & FY22 combined so the actual total not to exceed amount is $24,089,255.78. This new IGA contains five (5) activities/services and caps the indirect rate for four (4) of the five (5) activities at 10% and the fifth (5) activity, the Maricopa IDEAS, at full indirect rate. This Grant deviates from County Policy A2505 and does not allow for full indirect cost reimbursement, but a maximum of 10% indirect cost reimbursement as delegated in the Grant Agreement. MCDPH indirect rate for FY23 is 17.24%. Full indirect costs are estimated at $3,775,443.36 of which $2,189,932.34 is recoverable and $1,585,511.02 is unrecoverable. Program costs not covered by the grant will be subsidized by the MCDPH indirect cost pool. Departmental indirect rates are re-established at the beginning of each fiscal year and the future indirect rates will be collected at the corresponding rates. (C-86-23-078-X-00)

62. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES (ADHS) FOR COVID-19 VACCINATIONS Approve Amendment 1 to the cost reimbursement Intergovernmental Agreement (IGA), CTR062177, between ADHS and Maricopa County by and through its Department of Public Health (MCDPH), Office of Community Wellness and Health Promotion, provides funding for th e COVID-19 Vaccinations. The IGA provides vaccines to children and adults in accordance with recommendations of the CDC Advisory Committee on Immunization Practices (ACIP), prevents and controls Vaccine-Preventable Diseases (VPD). It is mutually agreed that the referenced IGA is amended as fellow: 1. Pursuant to the Terms and Conditions, Provision Six (6), Contract changes, Section 6.1 Amendments, Purchase Orders and Change Orders, the following are made under this Amendment One (1): 1.1.1 Exhibit C - 2 CFR 200.332 is revised and replaced 1.1.2 Exhibit E – 2 CFR 200.332 is revised and replaced Subsequently, this Amendment reduces the Indirect Rate Cost on Supplemental Flu Vaccination Activities (SAIF Funds) and Enhance VFC/COVID-19 Activiti es (IDEAS Funds) from 10% to zero percent (0%). All other Terms and Conditions of the original contract shall remain in full force and effect. The term of this Agreement is July 1, 2022, through termination date, June 30, 2024. (C-86-23-078-X-01)

69. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR IMMUNIZATION SERVICES Approve Amendment 3 to the Intergovernmental Agreement (IGA), CTR062177, between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), Office of Community Wellness and Health Promotion, provides funding for Immunization Services. The IGA provides voluntary vaccines to children and adults in accordance with recommendations of the CDC Advisory Committee on Immunization Practices (ACIP) to prevent and control Vaccine-Preventable Diseases (VPD). It is mutually agreed that the referenced IGA is amended as follows: Pursuant to Terms and Conditions, Provision Six (6), Contract Changes, Section 6.1., Amendments, Purchase Orders and Changes Orders, the following revision is made under this Amendment Three (3): 1.1. The Price Sheet is hereby revised and replaced; and 1.2. Exhibit F is hereby revised and replaced. All other Terms and Conditions of the original contract shall remain in full force and effect.
