98. APPLY FOR GRANT FUNDING OPPORTUNITY FROM CENTERS OF DISEASE CONTROL AND PREVENTION Approve MCDPH to apply for a Grant funding opportunity from the Centers for Disease Control (CDC) NOFO title “BOLD Public Health Programs to Address Alzheimer’s Disease and Related Dementias”. The anticipated funding and term date has yet to be determined. The estimated award date will be issued September 2023 with the final amount and terms. This is a competitive award, at this time, it has not been determined if it will be reoccurring. There is a cash or in-kind matching requirement of 30% during the cooperative agreement period. Match can be provided through additional funds by MCDPH and in-kind services. Should the agreement cease, ongoing contributions are not required. This grant allows for full indirect cost reimbursement. The indirect rate for (FY23) is 17.24%. Indirect costs are fully recoverable. The grant award is not a mandated function but provides a benefit to the citizens in Maricopa County by building an infrastructure and increase capacity in the field of Alzheimer’s Disease and related Dementia. 49. ACCEPT GRANT FUNDS FROM THE DEPARTMENT OF HEALTH AND HUMAN SERVICES CENTERS FOR CENTERS OF DISEASE CONTROL AND PREVENTION FOR ALZHEIMER’S AND DEMENTIA. Approve Notice of Award (NOA) 1 NU58DP007535-01-00, from the Department of Health and Human Services (DHHS), Centers for Disease Control and Prevention (CDC) and Maricopa County by and through its Department of Public Health (MCDPH) to address Alzheimer’s Disease and Related Dementias with an in the not-to-exceed amount of $250,000 per year. This budget period starts on September 30, 2023, and ends on September 29, 2024. The non-competing continuation award period begins on September 30, 2023, through September 29, 2028. This is a competitive grant award, and it will be reoccurring. This is the first time it has been awarded to the department. The department has an in-kind match requirement of $78,379.00 during the grant award period. The grant award does not require ongoing cash contributions for the next five years to maintain the program after the grant period end date. The Department of Public Health’s indirect rate for FY24 is 15.65%. Indirect costs are estimated at $33,830.52, all of which is recoverable. Departmental Indirect Rates are reestablished at the beginning of each fiscal year and the future Indirect Rate will be collected at the corresponding rates. The grant award is not a mandated function but provides a benefit to the citizens in Maricopa County for collaborative work with community partners and subject matter experts for create, maintain, or expand a coalition to inform and create a local strategic plan to address dementia and Alzheimer’s. This Non-Competing Continuation NOA was issued by the CDC on September 24, 2025. The total not-to-exceed amount for this NOA is $450,000.00, for the budget period September 30, 2025, through September 29, 2026. This is a partial NOA that brings the total funding for this budget period up to the not-to-exceed amount of $525,614.00. The original term of this agreement began on September 30, 2023, and will continue through September 29, 2028. This is a competitive grant award and is reoccurring. This is year three of the grant and MCDPH has an in-kind matching contribution requirement of $75,614.00 during the grant award period.
