4. APPOINTMENT TO THE SELF-INSURED TRUST FUND, BOARD OF TRUSTEES Approve the appointment of Tom Katsenes to the Self-Insured Trust Fund, Board of Trustees representing Supervisorial District 3. The term of the appointment will be effective as of the date of Board approval through April 30, 2013. (C-06-10-431-9-00)

5. CHANGE ORDER TO PARCOMM PROGRAM MANAGEMENT CONTRACT FOR THE DOWNTOWN COURT TOWER PROJECT AND FIRST AVENUE JAIL DEMOLITION AND PLAZA EXPANSION Approve Change Order #4 to Parsons Commercial (Parcomm) contract No. C-70-08-024-5-00 (FMD-08-020) in an amount not-to-exceed $365,000 to add one year of additional services (July 1, 2010 through June 30, 2011) for the Assistant Program Manager and an allowance for selection of an artistic design sub-consultant for the Superior Court Plaza redesign in connection with the Downtown Court Tower project (3325-07-380). Funding for this change order is split between the two projects: Downtown Court Tower Project (JUST) and First Avenue Jail Demolition and Plaza Expansion (FAJD) in the Five Year Capital Improvement Plan. This project is located in District 5. (C-70-08-024-5-03)

6. AGREEMENT WITH CITY OF PHOENIX FOR ARIZONA REVOCABLE PERMIT NO. RP-09021-23 Approve an Agreement between the City of Phoenix, (RP-09021-23) and Maricopa County through the Facilities Management Department, for the purpose of construction, use and maintenance of a Storm Water Treatment System and Guard Shelter Awning within the public right-of-way. This Agreement is non-financial and is effective from May 12, 2010 until revoked by either party. This Revocable Permit between the County and the City of Phoenix is required to complete portions of the Downtown Court Tower project (#3325-07-380). This project is located in District 5.
