78. IGA WITH TOWN OF GUADALUPE FOR COMMUNITY ACTION PROGRAM ACTIVITIES Approve an Intergovernmental Agreement (“Agreement”) between the Town of Guadalupe (“Contractor”) and Maricopa County, administered by its Human Services Department (“County”). The Contractor and County are collectively referred to here as the “Parties” and individually as a “Party.” The purpose of the Agreement is to provide Community Action Program (CAP) Community Initiative services in the Town of Guadalupe and surrounding areas. The Contractor shall distribute food boxes to enhance nutritional needs in the community as well as refer residents to County’s crisis case management services. The County shall provide the Contractor with $100,000 for services delivery; period of funding availability and term of the Agreement shall be retroactive to July 1st, 2022 through June 30, 2023. Also request authorization for the Chairman of the Board of Supervisors to approve and execute Administrative Change Orders upon the recommendation of the Human Services Director and Legal Counsel as identified in Section 1 – (General Provisions) 6.0 (Administrative Change Orders). 61. AMENDMENT TO IGA WITH THE TOWN OF GUADALUPE FOR COMMUNITY ACTION PROGRAM ACTIVITIES Approve Amendment No. 1 to the financial Intergovernmental Agreement between the Town of Guadalupe and Maricopa County, administered by its Human Services Department. The County contracts with the Town for the provision of Community Action Program (CAP) services in specific geographic areas and service boundaries. The purpose of Amendment No. 1 is to address the following: A. Extend the Agreement term from July 1, 2023 through June 30, 2024. B. Revise Section 1.0 (Purpose) as follows: The Contractor shall provide navigation services and program information/referrals to Town of Guadalupe residents. Detailed program activities identified in Section 3 (Work Statement). C. Revise Section 3 (Work Statement), by removing it in its entirety and replacing it with a new Scope of Work.: D. Revise Section 4 (Budget and Compensation), paragraph 1.5 (Operating Budget) by removing in its entirety and replacing the Operating Budget. The County shall provide the Contractor with a not to exceed amount of $107,254 for the period of July 1, 2023, through June 30, 2024. 51. AMENDMENT TO IGA WITH THE TOWN OF GUADALUPE FOR COMMUNITY ACTION PROGRAM ACTIVITIES Approve Amendment No. 2 to the financial Intergovernmental Agreement between the Town of Guadalupe and Maricopa County, administered by its Human Services Department. The County contracts with the Town for the provision of Community Action Program (CAP) services in specific geographic areas and service boundaries. CAP services include the delivery of Crisis Case Management and the coordination of services to assist low-income households in crisis situations and support economic mobility and self-supporting activities. The purpose of Amendment No. 2 is to address the following: A. Revise Section 1 (General Provisions) to add required clauses related to the Uyghur Forced Labor Prevention Act (UFLPA) B. Revise Section 4 (Budget and Compensation), Paragraph 1.0 (Budget), Subparagraph 1.2.3 (Funding), by adding Assistance Listing Number (ALN) 21.027 American Rescue Plan Act (ARPA) as a source of funding. The ARPA ALN is an existing source of funding in the Agreement and the overall Agreement budget is not impacted. This Amendment No. 2 shall be effective upon approval and signature by both Parties. The purpose of Amendment No. 3 is to address the following: A. Extend the Agreement term from July 1, 2024 through June 30, 2025. B. Revise and replace Section 3 (Work Statement) in its entirety. C. Revise and replace Section 4 (Budget and Compensation), Paragraph 1.5 (Operating Budget). The County shall provide the Town with $107,245, period of funding availability is July 1, 2024, through June 30, 2025. D. Update required Agreement clauses. The County provided the Contractor with $107,254 for Fiscal Year 2025 for service delivery. This funding is made available to the County through an Intergovernmental Agreement with the Arizona Department of Economic Security (ADES), comprised of Community Services Block Grant (CSBG), Low Income Home Energy Assistance Program (LIHEAP), American Rescue Plan Act (ARPA) and County General Funds. The term of the Agreement is July 1, 2024, through June 30, 2025 The purpose of Amendment No. 4 is to address the following: A. Increase funding in an amount not to exceed $61,776, for a new Fiscal Year 2025 total funding amount of $169,021. B. Update County Point of Contact. C. Update required Agreement Clauses. D. Revise and replace Section 4 (Budget and Compensation), Paragraph 1.5 (Operating Budget). The County shall provide the Town with $169,021 for service delivery; the period of funding availability is July 1, 2024, through June 30, 2025. AMENDMENT TO IGA WITH TOWN OF GUADALUPE FOR COMMUNITY ACTION PROGRAM ACTIVITIES Approve financial Amendment No. 5 to the Intergovernmental Agreement (IGA) between the Town of Guadalupe and Maricopa County, administered by its Human Services Department (County) Community Resilience Division. The County and the Contractor collectively are referred to as the “Parties.” The County contracts with the Town for the provision of Community Action Program (CAP) services in specific geographic areas and service boundaries. CAP services include the delivery of Crisis Case Management and the coordination of services to assist low-income households in crisis situations move closer to economic self-sufficiency. The County provided the Contractor with $169,021 for Fiscal Year 2025 for service delivery. This funding is made available to the County through an Intergovernmental Agreement with the Arizona Department of Economic Security (ADES), under contract DI20-002264, comprised of Community Services Block Grant (CSBG), Low Income Home Energy Assistance Program (LIHEAP), American Rescue Plan Act (ARPA) and County General Funds. The term of the Agreement is July 1, 2024, through June 30, 2025. The purpose of Amendment No. 5 is to address the following: A. Extend the Agreement term from July 1, 2025, through June 30, 2026. B. Retroactive to July 1, 2024, Fiscal Year 2025, as amended, in accordance with 2 C.F.R. 200.331(a), the Town of Guadalupe shall be identified as “Subrecipient” for the purpose of carrying out the activities identified in the Work Statement in Section 3. C. Revise the Agreement funding by an amount not to exceed $107,254 for Fiscal Year 2026. The funding sources for this Amendment may include but are not limited to the following: 93.569 Community Services Block Grant (CSBG), 93.568 Low Income Home Energy Assistance Program (LIHEAP), 21.027 Coronavirus State and Local fiscal Recovery Funds (SLFRF). D. Update County point of contact. E. Add required Agreement clauses. F. Revise Section 2 (Special Provisions) Paragraph 4.0 (System for Award Management), to add subparagraph 4.4. G. Revise and replace Section 3 (Work Statement) in its entirety. H. Revise Section 4 (Budget and Compensation), Paragraph 1.5 (Operating Budget) and replace the Operating Budget by incorporating a new attached Operating Budget into the Agreement. The County shall provide the Subrecipient with a not to exceed amount of $107,254 for the period of July 1, 2025, through June 30, 2026 (Fiscal Year 2026). Unexpended funds from Fiscal Year 2025 shall not be available for expenditures in Fiscal Year 2026. I. In accordance with 2 CFR 200.332(b)(1)(xii), the County shall provide the Subrecipient with the FY2026 final expenditure dollar amounts made available under each federal award program (according to Assistance Listing Number (ALN) within 60 days after June 30, 2026.
