1. APPOINTMENT FOR LEGISLATIVE DISTRICT 13 HOUSE OF REPRESENTATIVES Pursuant to A.R.S. § 41-1202, receive the names of the three qualified electors as candidates to fill the vacancy for the District 13 Representative seat as submitted by the Legislative District 13 Citizens Committee regarding the LD-13 Representative vacancy. The three names submitted are: Lorenzo Sierra Martin J. Quezada Paul Valach The Board of Supervisors will vote to appoint one of the candidates from the list to fill the vacancy created by the resignation of Representative Richard Miranda. 2. CHANGE ORDERS TO GMP 3 WITH GILBANE BUILDING CO. FOR THE DOWNTOWN COURT TOWER Approve and authorize Change Order No. 13 to GMP #3C and Change Order No. 8 to GMP #3D both of which are with Gilbane Building Company (C-70-09-023-5-00) for construction of the Downtown Court Tower Facility project (3325-07-380) to extend the contract end date from February 9, 2012 to March 31, 2012 and authorize the Director of Facilities Management to terminate the contract prior to March 31, 2012 upon verification that all work is completed. This facility is located in District 5. Funding for this change order is budgeted in FY2012. (C-70-12-002-3-00)

3. CHANGE ORDER NO. 15 TO GMP 4 WITH GILBANE BUILDING CO. FOR THE DOWNTOWN COURT TOWER Approve and authorize Change Order No. 15 to GMP #4 with Gilbane Building Company (C-70-09-028-5-00) for construction of the Downtown Court Tower Facility project (3325-07-380) to extend the contract end date from February 9, 2012 to March 31, 2012 and authorize the Director of Facilities Management to terminate the contract prior to March 31, 2012 upon verification that all work is completed. This facility is located in District 5. Funding for this change order is budgeted in FY2012. (C-70-12-003-3-00)

4. CHANGE ORDER NO. 13 TO GMP 6 WITH GILBANE BUILDING CO. FOR THE DOWNTOWN COURT TOWER Approve and authorize Change Order No. 13 to GMP #6 with Gilbane Building Company (C-91-10-146-5-00) for construction of the Downtown Court Tower Facility project (3325-07-380) to extend the contract end date from February 9, 2012 to March 31, 2012 and authorize the Director of Facilities Management to terminate the contract prior to March 31, 2012 upon verification that all work is completed. 5. CHANGE ORDER NO. 20 TO GMP 5 WITH GILBANE BUILDING CO. FOR THE DOWNTOWN COURT TOWER Approve and authorize Change Order No. 20 to GMP #5 (C-70-10-005-5-00) (FMD-10-010) with Gilbane Building Company for construction of the Downtown Court Tower Facility project (3325-07-380) to extend the contract end date from February 9, 2012 to March 31, 2012 and authorize the Director of Facilities Management to terminate the contract prior to March 31, 2012 upon verification that all work is completed.
