Marquoit as a Superior Court Judge Pro Tempore in accordance with Arizona Revised Statutes § 12-141. The appointment will be for the period commencing October 2, 2017 through June 30, 2018. This serves the interest of judicial economy and promotes sound caseflow management. Court Commissioner Thomas L. This Agreement is funded by a grant from the DHS (Award Number 2006-ST-091-000003-12) with the purpose of continuing funding for the twelfth year of the BioWatch program. The BioWatch program includes an ambient air monitoring network to sample air pollution in a variety of settings, assess health and welfare effects, assist in determining sources of air pollution; and to carry out the provisions of the US EPA, the Federal Clean Air Act and Amendments, Arizona Revised Statutes, and County Air Pollution Control Regulations that are intended to correct, remove, minimize or prevent air pollution within Maricopa County. The Agreement is effective from September 1, 2006 until June 30, 2018, while the budget is retroactively effective from July 1, 2017 until June 30, 2018. Project budgets are applied for annually. The Agreement allows the Air Quality Department $135,698 in indirect costs with direct charges of $465,541. The recoverable indirect cost of administering this grant is $135,698; the non-recoverable indirect cost is $0. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore, expenditure of the funds is not prohibited by the budget law. A budget adjustment is not required. (C-85-16-012-G-01)

The IGA term is September 1, 2017 through May 31, 2020. • Replace Attachment A with the Revised Attachment A. Tobacco Use is the number one preventable cause of death and disease in Arizona. In 1993, there were over 70,000 middle and high school youth using tobacco. Cigarette smoking during pregnancy accounts for 20 – 30% of low birth weight babies, up to 14% of pre-term deliveries, and about 10% of all infant deaths. In 1995, as a result of the Tobacco Tax Initiative, the Arizona Legislatures authorized the Arizona Department of Health Services (ADHS) to develop and fund tobacco prevention and cessation programs. The Maricopa County Department of Public Health (MCDPH) is funded by ADHS to identify effective strategies and implement school-based tobacco use prevention services for Maricopa County as part of the MCDPH comprehensive tobacco control program. 52. RESCIND AND APPROVE AMENDMENT TO CONTRACT WITH BANNER HEALTH FOR COMMUNITY HEALTH NEEDS ASSESSMENT REPORT Rescind an action taken on April 12, 2017 for item (C-86-15-050-3-01) Amendment no. 1, Contract for services between Banner Health (BH), an Arizona nonprofit corporation and Maricopa County by and through its Department of Public Health (MCDPH) in order to continue participating in the Community Health Needs Assessment (CHNA). The amendment was in the amount of $300,000 and to extend the end date through June 30, 2020. BH requested to replace the Amendment no. 1 document with their Amendment no. 1 version with additional clauses and did not sign the original amendment. Also, approve the revised First Amendment to the Contract (0101-12-46149 A1) between Banner Health (BH), an Arizona nonprofit corporation and Maricopa County by and through its Department of Public Health (MCDPH) in order to continue participating in the Community Health Needs Assessment (CHNA). The amendment is in the amount of $100,000 per year, for 3 years, for a total not-to-exceed amount of $300,000. The term of this Contract is extended through June 30, 2020. This Amendment also includes Items 1. Amendment of the Contract; 2. Contract Effective; 3. Precedence of Amendment; 4. Capitalized Terms; 5. Incorporation; 6. Counterparts; and 7. Corporate Authority. The Department of Public Health’s FY2017-18 indirect rate for fee fund is 12.6%; indirect costs are estimated at $33,570 all of which is fully recoverable. Previously, the Department of Public Health would (at no charge) provide hospitals and Federally Qualified Health Centers (FQHCs) with extensive data requests and reports to support CHNA requirements. As requests for data became more extensive, PH was unable to meet the demand while also meeting other Department obligations. (C-86-15-050-3-02)

73. REPLACEMENT WARRANTS – CHECK ENFORCEMENT PROGRAM Approve the issuance of replacement warrants for county warrants related to the Check Enforcement Program that have been outstanding for more than one year. Necessary affidavits have been filed with the Board. The Maricopa County Attorney’s Office (MCAO) Check Enforcement Program endeavors to make victims of bad check writers financially whole by pursuing restitution and holding offenders accountable. This service is offered free of charge to individuals and merchants who have gone unpaid due to insufficient funds. When restitution has been received, the MCAO Check Enforcement Program issues a warrant to the victim. Unfortunately in some cases, warrants are returned to MCAO or remain uncashed because of an address change, an incorrect address on file or changes in merchant ownership. To date, the MCAO Investigations Division has located current contact information for 55 victims who were issued warrants prior to June 2015 that have not been negotiated. MCAO requests the issuance of replacement warrants to these victims in the total amount of $40,813.05. These funds will directly impact victims of bad check writers by making them financially whole.
