WELLNESS INCENTIVE REQUIREMENTS FOR PROGRAM YEARS 2016-2017 AND 2017-2018 Approve the Wellness Incentive Requirements for Program Years 2016-2017 and 2017-2018, specifically, July 1, 2016 through June 30, 2017 and July 1, 2017 through June 30, 2018, as outlined on the attached Exhibit A. The basic requirements for employees to earn wellness premium reduction incentives have not changed since 2008-09. The proposed changes will move the program forward from incentivizing employees to be aware of their actual or potential health risks to incentivizing employees for taking positive steps to mitigate or prevent health risks. These changes are important to help manage the County’s cost of employer-sponsored health care, which will total $134,328,061 in FY 2015-16, and are important to employee productivity and engagement. The changes in incentive design follow current best practices, and were developed by County staff in consultation with the County’s wellness vendor, StayWell. The total amount of available wellness premium reduction incentives remains unchanged for Program Years 2016-2017 and 2017-2018, at up to $60.00 per month ($30 per pay period) for employees enrolled in a Maricopa County employee medical benefits plan. However, the way in which employees may qualify for the incentives will change. Participation remains voluntary; enrollment in a Maricopa County medical benefits plan is not contingent upon whether an employee qualifies for an incentive. Approval of this agenda item will allow employees to continue their 2015-16 wellness incentives in Program Year 2016-2017 if criteria are met. To earn their full incentive in Program Year 2016-2017, employees must complete a biometric screening, a health assessment, and pledge that they are tobacco free (or complete an alternative tobacco cessation program). Screening for tobacco use will no longer be required. During Program Year 2016-2017, employees will also have the opportunity to qualify for Program Year 2017-2018 wellness incentives. Biometric screenings, health assessments and the tobacco free pledge (or alternative activities) will count towards the overall points necessary to qualify for the wellness incentive for Program Year 2017-2018; however, to earn incentives employees will also be required to complete other wellness-related activities from the list of options (set forth in Table 1 of Exhibit A) to meet the necessary amount of points to qualify for the full amount of the monthly wellness premium reduction incentives. Employees must earn the necessary points to qualify for Program Year 2017-2018 wellness premium reduction incentives by May 31st, 2017. With the exception of new employees, employees who do not meet the deadline date of May 31st, 2017 will not receive the incentives in 2017-2018. New employees will be allowed to qualify for the wellness premium reduction incentives under a modified time frame, as further described in Exhibit A. (C-39-16-021-2-00)

30. EMPLOYEE BENEFIT TRUST – BENEFIT PREMIUM HOLIDAY - PAY PERIOD END 6/5/2016 Approve the following: 1. Implement a premium holiday for self-insured employee medical, pharmacy and behavioral health plans for the pay period ending June 5, 2016 (reflected in paychecks to be issued June 15, 2016). The premium holiday will affect both employee and employer contributions. The estimated savings to employees is $ 571,715 and the estimated savings to the employer are $ 5,203,899, a combined total of $5,775,614. The FY 2016 Budget was developed according to this plan. 2. In the event that Net Position in the Maricopa County Employee Benefits Trust (excluding Short-Term Disability funds) is reduced to deficit as of June 30, 2016, the shortfall will be eliminated by a fund transfer from the General Fund. Such a transfer from the General Fund may occur in FY 2016-17. 3. Designate $5,800,000 in the General Fund Reserves FY 2017 of the FY 2017 Budget as a contingency to cover a shortfall in the Benefits Trust, should the fund balance drop below zero. This amount may be reallocated once additional revenue accumulates to restore reserves in the Benefits Trust to minimum recommended levels. This action will reduce the Net Position of the Employee Benefits Trust by reducing revenue that would otherwise be collected. The premium holiday will be implemented by not charging employee and employer premiums for medical, pharmacy and behavioral health plans for all employees that have an active “A” status or leave status of “L” or “P” at the time of the premium holiday. This action will reduce Benefit Trust revenues but will not eliminate the benefit cost allocation to departments for the June 5, 2016 pay period. The cost allocation was budgeted to be lower than actual benefit premium charges and thus the departments have already benefited from receiving a discounted cost allocation. (C-39-16-020-2-00)

63. IGA WITH CITY OF SURPRISE FOR THE EXCHANGE OF SERVICES (ENTENTE) Approve the Intergovernmental Agreement between Maricopa County and the City of Surprise for the exchange of services or reimbursement of services (Entente). The term of the Agreement will not exceed five years unless extended by appropriate actions by the County and the City. Either party may terminate this Agreement at any time prior to the expiration date of this Agreement by furnishing the other party with a thirty (30) day written notice. The purpose of this IGA is to identify and define both Parties’ respective roles in the execution of the responsibilities of specific maintenance tasks in areas that cross jurisdictional boundaries. The Agreement provides the opportunity to focus on specific maintenance tasks or emergency highway projects and the availability of the most economical resources by extending beyond jurisdictional or geographical boundaries. The application of the Agreement will benefit areas in the County and the City where certain types of routine maintenance or emergency highway project activities could be completed by the other party based on scheduled work within the area. Individual tasks performed by either party will be executed upon completion of a Letter of Agreement (LOA) approved and signed by the County Department of Transportation Director and the City Manager, or their designees. If the value of goods or services exchanged by the Parties is not of equal value, the total aggregate reimbursement to the respective Party will not exceed $250,000 per fiscal year. If no services are exchanged, there is no financial impact. An accounting mechanism will be utilized to track each LOA, as applicable, to identify specific tasks and services and to ensure the exchange of these services is of similar value. 64. INGRESS AND EGRESS EASEMENT: SALT RIVER PROJECT Approve and execute an Ingress & Egress Easement to Salt River Project over the westerly 20 feet of 95th Avenue lying between Missouri Avenue and San Miguel Avenue as situated in Pendergast West sub-division as recorded in Maricopa County Recorder’s Office Book 143, Page 4. A land right for a tail water ditch held since July 10th 1937 by the United States of America through the Bureau of Reclamation occupied a portion of 95th Avenue which was established per the Pendergast West sub-division plat. It was determined that the ditch was only partially located within said land right and partially on property to the west of the subdivision. Subsequently a replacement land right was granted to the United States of America by the private landowner to accommodate the ditch in situ. This land right was recorded in Maricopa County Recorder’s Office 2016-0138367. In order to facilitate that replacement land right granted by the private property owner, Salt River Project, the Operator of this irrigation system, required an Ingress & Egress right over the westerly 20 feet of the 95th Avenue alignment so that the ditch could continue to be maintained in situ.
