GRANT FUNDS FROM ARIZONA STATE DEPARTMENT OF HOMELAND SECURITY FOR RENEWAL OF EMERGENCY PLANS SOFTWARE Authorize Emergency Management to apply for grant funds from Arizona State Department of Homeland Security, (FFY21 SHSGP), in the not-to-exceed amount of $85,000 for the MaricopaRegionPrepares.com subscription renewal. This is a Continuity of Operations Planning (COOP) software package for all jurisdictions within Maricopa County. The grant period for this grant is October 1, 2021 through September 30, 2022. The Maricopa County Department of Finance has calculated the Department of Emergency Management's composite indirect cost rate at 10% or $8500. The Department will not be recovering indirect costs off of this grant. The amount of unrecoverable indirect cost is $8500. Emergency Management will absorb these indirect costs within their general budget. The grant award is non-recurring, the department has to apply every year however, we have received the grant for the past four years. The department does not have an in-kind match requirement for this grant. The grant award is not a mandated function but it is a department strategic goal to have all county departments with a viable Continuity of Operations Plan (COOP). The grant is awarded through the Arizona State Homeland Security Department-State Homeland Security Grant Program (SHSGP), which is a competitive grant program. Maricopa County jurisdictions compete for projects based off the State grant initiatives. Maricopa County has spear headed the grant for the Emergency Operations Plans (EOP)/COOP software project for all county departments and jurisdictions within Maricopa County. Each year Maricopa County applies for the recurring cost of the software maintenance and training funds. Maricopa County does not have to fund any additional costs for the program. The grant is intended to renew the MaricopaRegionPrepares.com subscription. This software tool benefits Maricopa County in developing Emergency Operations Planning (EOP), Continuity of Operations Planning (COOP), Continuity of Government (COG) Planning for local government. The Department was awarded a grant for the software in 2015, we are applying for a grant to cover the cost associated with the maintenance of the system. Funding for this agreement is provided by a grant from the State of Arizona Department of Homeland Security in the amount not to exceed $85,000. The overall grant budget will be adjusted as necessary to accommodate this grant through future reconciliation. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the funds is not prohibited by the budget law. The approval of this action does not alter the budget constraining the expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. (C-15-21-010-X-00)

The (CDBG-CV) funding allocation amount is $2,922,767; and 2. Approve the submittal of Amendment No. 3 to the FY2019-20 Annual Action Plan (AAP) to HUD. The AAP identifies the activities to be conducted with the CDBG-CV funds as identified in Attachment A: a. Public Services – Homelessness; Rental, Mortgage, Utility, Deposit Assistance for Urban County areas; and Program Administration. 3. Also request authorization and approval for the Human Services Department Director to sign any documents required by HUD for this funding allocation. The County received notification from HUD that a second allocation of CDBG-CV CARES Act funding, also referred to as CDBG-CV3 in HUD documents, was awarded to the County. These special CDBG-CV funds are to be used to prevent, prepare for, and respond to the coronavirus pandemic (COVID-19). The funds must address the needs of low-and moderate-income persons and the development of partnerships between all levels of government and the private for-profit and nonprofit sectors related to COVID-19 pandemic response. The HUD notification lists that States and local governments may reimburse costs of eligible activities incurred for pandemic response regardless of the date” and does not list an end date for the period of availability. HUD notified the County that it would need to make a Substantial Amendment to its 2019 Annual Action Plan in order to add these new funding sources. Further clarification and direction are provided in the HUD notifications and will be subject to any additional applicable regulatory waivers, funding flexibilities, or conditions as granted or allowed by HUD. Approval of this item shall not impact the County General Fund. FY 2021: $2,922,767 Revenues & Expenditures Supervisory District: All (C-22-16-032-G-08)

62. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR CLINICAL SERVICES CAPACITY PROGRAM Approve an amendment to the Intergovernmental Agreement (IGA) Contract No. CTR043100, HIV Surveillance Capacity Expansion Program, between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH). The Price Sheet is in the not-to-exceed amount of $192, 000 for the budget period January 1, 2021 through December 31, 2021. The term is April 1, 2019 to March 31, 2024. This is a new IGA with a 5 year term. It may reoccur but it is uncertain. Neither cash nor in-kind match is required and the need for ongoing cash contributions is not expected. The grant award is not a specific mandated function. This was not a competitive process. The Department of Public Health’s indirect rate for FY 2020-21 is 18.8%. Total indirect expenses are estimated to be $30,384 all of which is recoverable. All program costs are allocated to the grant so there should be no additional burden on the department’s operating budget. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-19-057-3-03)

Authorize the Board Chairman to sign all documents related to these grant funds, as applicable. The project period is February 2, 2021 through June 30, 2021. The grant is a part of a larger ‘Cost Evaluation Project’. County will report site-level data related to the administration of express and non-express services with findings to be analyzed by NACCHO and CDC. This was a non-competitive award with no cash or in-kind match required. This is not a mandated portion of STD services but provides a benefit to our citizens by working to discover efficiencies in the provision of related services. Should the grant cease, on-going contributions will not be required. The grant allows for the full indirect rate of 18.8%. Indirect cost is estimated to be $1,582 all of which is recoverable. All program costs are allocated to the grant so there will not be additional burden on the department’s operating budget. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore, expenditure of the funds is not prohibited by the budget law. Approval of this action does not alter the budget constraining the expenditure of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. (C-86-21-123-X-00)

69. RFP CONTRACT CONSOLIDATION FOR MASS IMMUNIZATION SERVICES AND ADMINISTRATION Currently, there are nine (9) active contracts developed and completed as needed in response to immunization needs and COVID-19. The contracts were procured under three (3) different RFP solicitations, thereby having three (3) different contract numbers. This has led to some confusion therefore it is the intent of MCDPH to consolidate all nine (9) contracts under the initial RFP contract number of 200234. The initial contracts were entered into under the auspices of item C-86-20-552-M-00, approved June 8, 2020, which authorized funding for the provision and administration of vaccines. The RFPs are as follows: PH RFP 200234: 5 contracts; PH RFP 210132: 2 contracts; PH RFP 210148: 2 contracts. Approve and authorize the re-naming of the four (4) contracts that were procured later under PH RFP 210132 and PH RFP 210148 to be named PH RFP 200234 and consolidate all contracts under the same solicitation number in order to alleviate confusion and expand transparency. The solicitation is currently open again for a 4th time in order to locate additional vaccine providers and it is the intent of MCDPH to add these new vendors as well to the existing PH RFP 200234 contract. These agreements will be funded by the CARES Act; the use of these funds has been extended through 12/31/2021. Should funding be discontinued, and alternate funding not be available, Section 4.0 of the Contract, Availability of Funds will apply, allowing for a change or termination of the contract. IGA WITH THE CITY OF PEORIA FOR THE EXCHANGE OF SERVICES (ENTENTE) Approve the Intergovernmental Agreement between Maricopa County and the City of Peoria for the exchange of services or reimbursement of services (Entente). This Agreement shall become effective as of the date it is approved by the Maricopa County Board of Supervisors and remain in full force and effect until either Party terminates it. Either Party may terminate this Agreement at any time before the expiration date of this Agreement by furnishing the other Party with a thirty (30) day written notice. The purpose of this Intergovernmental Agreement is to identify and define both Parties' respective roles in the execution of the responsibilities of specific maintenance tasks in areas that cross jurisdictional boundaries. The Agreement provides the opportunity to focus on specific maintenance tasks or emergency highway projects and the availability of the most economical resources by extending beyond jurisdictional or geographical boundaries. The Agreement's application will benefit areas in the County and the City where the other Party could complete specific types of routine maintenance or emergency highway project activities based on scheduled work within the area. Individual tasks performed by either Party will be executed upon completing a Letter of Agreement (LOA) approved and signed by the County Department of Transportation Director and the City Manager, or their designees. If the value of goods or services exchanged by the Parties is not of equal value, the total aggregate reimbursement to the respective Party will not exceed $250,000 per fiscal year. If no services are exchanged, there is no financial impact. An accounting mechanism will be utilized to track each LOA, as applicable, to identify specific tasks and services and ensure the exchange of these services is of similar value. MC1-377 - Increase the maximum allowable amount that departments can spend on conference facilities from $10,000 to $15,000. These limits have not been changed in over ten years and are no longer practical. MC1- 503.G. 2 Modify the maximum value of a single job order contact assignment from $1 million to $2 million. In response to requests from the Directors of the Flood Control District, Transportation and Facilities Management, staff recommends an amendment to the single work assignment limit on a job order contract from $1 million to $2 million. Due to rising construction costs, this change is necessary in order to continue to use the JOC project delivery method effectively. MC1-346.D & E Increase nominal value from $5,000 to $10,000 in order to conform with 2 C.F.R.§200.67 (Uniform Guidance) and Arizona Administrative Code § R2-7- D302. Following the Public Hearing, the Board is requested to adopt the proposed amendments to the Code. Upon Board approval, this item will become effective February 24, 2021. (C-73-21-067-X-00)
